White Plains Hospital Job DescriptionTitle: WPH Cares Clinical PharmacistJob Code: [Insert Code]Department/Location: Remote/On-site Hybrid - WPH CaresFLSA Status: ExemptReports to: Medical Director / Clinical Manager, WPH Cares Position SummaryUnder the supervision of the Medical Director of WPH Cares, the WPH Cares Clinical Pharmacist will collaborate with healthcare providers in an outpatient setting to optimize pharmacological therapy. This role involves providing evidence-based pharmacotherapeutic recommendations to prescribers, with a focus on improving patient outcomes, enhancing medication safety, preventing hospital readmissions, managing high-risk populations and promoting cost-effective therapy. The pharmacist will work closely with interdisciplinary teams to assess, monitor, and modify medication regimens, ensuring they align with current guidelines and patient-specific needs. They will also provide education and training for healthcare professionals involved in patient care and contribute to the development of telehealth-based pharmacy services.Essential Functions and ResponsibilitiesUnderstand and adhere to the WPH Performance Standards, Policies, and Behaviors.Perform comprehensive medication reviews and medication reconciliation through telehealth and occasional in-person visits.Provide recommendations to prescribers regarding optimal medication regimens, focusing on evidence-based guidelines (e.g., GDMT) for chronic conditions such as heart failure, COPD, diabetes, hypertension and others. Ensure medications are tailored to individual patient needs, considering factors such as comorbidities, drug-drug interactions, and patient preferences.Assess the appropriateness of drug therapy, including dose, route, frequency, and duration, adjusting as necessary to improve therapeutic outcomesProvide patient education on medication adherence, safety, and appropriate use, including OTCs and herbal supplements.Collaborate with physicians, healthcare providers, and the WPH Cares team to develop and implement care plans, aiming to optimize medication regimens.Promote clinical pharmacy development by providing leadership and education to pharmacy staff, including telehealth-based interventions.Identify and manage drug therapy problems, including inappropriate therapy, safety concerns, and cost-effectiveness issues.Conduct follow-up evaluations and consultations to ensure clinical recommendations are followed and goals of therapy are achieved.Assist with data collection, analysis, and reporting related to medication management and patient outcomes.Foster collaboration and trust among colleagues and departments, ensuring a smooth transition of care between hospital, telehealth, and home settings.Promote a culture of cost containment and efficiency in medication therapy.Participate in interdisciplinary team meetings, case discussions, and process improvement initiatives to enhance patient care. Maintain compliance with state and federal pharmacy laws, hospital policies, and telehealth protocols. Complete relevant competencies and training as required by the Director of PharmacyCollaborate with inpatient clinical pharmacists to provide continuity of care Perform other related duties as assigned by the Medical Director and Clinical Manager of WPH CaresEducation & Experience RequirementsDoctor of Pharmacy (Pharm.D.) from an ACPE-accredited school of pharmacy.Active and unrestricted New York State pharmacist license.Minimum of 2 years of experience in comprehensive medication management or ambulatory care (excluding clinical rotations).Experience in a telehealth or hybrid healthcare setting preferred.Certification in Board of Pharmacy Specialties (BPS) or Board Certified Ambulatory Care Pharmacist (BCACPs) (preferred).Certification in New York State Collaborative Drug Therapy (CDTM) is preferred Familiarity with federal and state pharmacy laws, telehealth protocols, and hospital-based pharmacy operations.Core CompetenciesStrong verbal and written communication skills, including patient engagement via telehealth.Proven ability to work collaboratively with healthcare teams and manage patient care remotely.Independent judgment and decision-making skills.Ability to educate patients and staff on medication management, adherence, and safety.Proficient in using telehealth platforms, EHR systems, and pharmacy management software.Adaptability to work in a dynamic hybrid role with both remote and on-site responsibilities.Ability to retain composure under stress and prioritize tasks effectively.Demonstrates cultural sensitivity and the ability to engage with diverse patient populations.Physical/Mental Demands & Work EnvironmentAbility to perform duties in both a home office and in-person hospital or clinic setting.Frequent use of telehealth platforms and technology for patient consultations.Ability to focus on detail-oriented tasks and make clinical decisions with minimal supervision.Occasional travel required to hospital or clinic locations for in-person patient consultations or meetings.Must be physically able to remain stationary or move about within office and hospital environments.May be exposed to chemicals necessary for pharmacy operations; all safety precautions as outlined in the hospital's SDS database must be followed.Primary Population ServedCheck appropriate box(s) below:•Neonatal (birth - 28 days)•Infant (29 days - less than 1 year)•Pediatric (1 - 12 years)•Adolescent (13 - 17 years)•Adult (18 - 64 years)•Geriatric (> 65 years)•All populations
