HYBRID POSITION. 3 DAYS REMOTE AND 2 DAYS LOCATED ON SITE IN GREENWOOD. NO HOLIDAYS, NO WEEKENDS. WHAT YOU CAN EXPECT- Medical Team Collaboration Collaborates with allied health disciplines to provide quality patient care. Provides clinical consultation and clarification concerning issues in drug therapy, medication reconciliation, patient care, and transitions of care medication needs. Attend rounds, patient care meetings or other similar activities as warranted.
- Staff and Patient Consultation Consults with and advises other Healthcare professionals and patients on matters pertaining to selection, procurement, distribution, and safe and appropriate use of medications.
- Staff Education Coordinates and delivers ongoing staff education for clinical staff pharmacist. Ensures that professional competence of clinical pharmacist staff is maintained through educational opportunities.
- Student and Resident Supervision Directs and precepts students and/or residents.
- Staff Education Leads cost effective formulary management initiatives. Leads or participates in formulary protocol/policy development and implementation. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.
- Staff Education Reviews adverse events related to the medication use process, recommends improvements to the medication use process as needed, and ensures improvements are implemented.
- Staff Education Reviews literature and publications in area of specialization and recommends improvements to the medication use process.
- Medication Order Review Reviews prescriptions and medication orders, checks for appropriateness and provides information needed to properly and safely administer the medication. Recognizes when orders are inappropriate and takes responsibility to assure that the order is modified promptly and correctly. Documents interventions. Reports adverse drug reactions and medication errors in the event reporting software.
- Local and System Collaboration Attends and participates in local and system-wide committees to improve the medication use process.
- Medication Policies Drafts, reviews, and revises medication related policies, procedures, guidelines and protocols.
- Clinical Pharmacotherapy Services Provides clinical pharmacy services as outlined in approved hospital policies, including but not limited to, pharmacokinetic consults, renal dosing, parenteral nutrition management, anticoagulation management, neonatal/pediatric/geriatric dosing management, route optimization, antimicrobial stewardship, and reviewing medications at transitions of care.
QUALIFICATIONS- Required Bachelor's Degree
- Preferred Doctorate
- 1 year PGY1 Pharmacy Residency Required
- 3 years In area of specialization Required
- 1 year PGY-2 Pharmacy residency Preferred
- Registered Pharmacist (RPh) - State Licensing Board
- Board Certified in applicable area of specialty or pharmacotherapy (if specialty area not available) - . Required within 24 months
