Job Summary
We are looking for individual to join our client s team as Licensed Master Social Worker. Under supervision provides Social Work services to patients attending the Outpatient Mental health/Chemical dependency service. Engages in independent decision-making about psychosocial needs, provides psychosocial services to individuals and their families, and provides formal and informal meetings regarding patient and family communication and decision making. Provides direct patient care as well as consultative services to assure the delivery of state-of- the-art care.
Job Description
- Completes Social Work Comprehensive Assessment for all patients asking for assistance, referred by others or in need of social work services. This is completed in accordance with departmental high/moderate risk criteria policy.
- Conducts interviews with patients and/or family members(s)/significant others(s) to determine current concerns and continuing care needs.
- Completes all appropriate social work documentation within designated time frames.
- Provides a hand-off of psychosocial information to the next worker for patients transferring to other units to ensure continuity of care.
- Provides supportive counseling, crisis intervention, and grief counseling to patients and their families/significant others as appropriate.
- Assists patients with adjustment to treatment, including problems of depression, anxiety, denial and/or anger, noncompliance with medical regimen.
- Devises/implements a social work treatment/continuing care plan (discharge planning) involving the patient, family/significant other and the interdisciplinary team. Documents this planning in the patient's medical record in accordance with the departmental policy.
- Provides information, referral, and advocacy to community agencies
- Advocates and serves as a liaison for patients with community agencies.
- Coordinates individual interdisciplinary case conferences as needed.
- Provides task supervision to caseworkers when designed.
- Provides direct/task supervision to social work students when designated.
- Leads education and support groups for patients and/or families/significant others as appropriate.
- Identifies, evaluates, and designs proper safety plans for victims of crimes/abuse, i.e: child abuse, elder abuse, domestic violence, rape and sexual assault.
- Maintains and completes statistics of case activities as outlined in the departmental policy.
- Monitors his/her performance by reviewing medical records for appropriate documentation.
- Attends individual, unit, departmental and intra/inter hospital meetings as mandated by the supervisor and/or department director.
- Assists/participates in presentations, training and in-services as required.
- Participates in the department's cross-training program.
- Coordinates and executes discharge plans for hospital patients.
- Performs other duties assigned by supervisor as needed.
Skills Required
- One-year LMSW experience
- Required - Experience working with severe mentally ill patients Will be working in ACT department
- Skills include - rehabilitation and support functions; assistance in treatment; provision of substance abuse services; providing education, support, and consultation to families; and provision of crisis-intervention services.
Education/Training/Certifications
- Master s Degree
- Valid Licensed Master of Social Work (LMSW), license issued by NYSED, Primary Source Verification required.

- Malpractice Insurance - All clinical workers are required to have active individual Medical Malpractice. A copy must be provided prior to start of assignment. All clinical workers licensed by the state of NY are required to provide.
