DescriptionCLINICAL SOCIAL WORKER (LCSW-C) JOB SUMMARY The Clinical Social Worker provides comprehensive social work services for the patient population served and care management services to those patients in clinical programs.ESSENTIAL FUNCTIONS OF POSITION
- Psychosocial assessment of families
- Participation in the development of an interdisciplinary treatment plan
- Provides educational, supportive and counseling services to families
- Coordination of services with community agencies
- Participating in discharge planning, and patient post discharge follow-up.
- Achieve high quality and cost effective patient outcomes across the continuum of care
- Functions with a focus on patient safety, customer service, and family centered care
- Participates in performance improvement initiatives for the department
POSITION REQUIREMENTS Educational Requirements Masters Degree in Social Work from a University accredited by the Council on Social Work Education.Licensure, Certification and RegistrationCurrent license to practice Social Work in the State of Maryland. LCSW-C required. Work Orientation and ExperienceFive years experience as a Social Worker in an acute or rehabilitation hospital with pediatric experience preferred. Experience in child welfare, family service, or mental health setting, is also desirable. Experience with organization and coordination of care for complex families; discharge planning, interagency coordination, case management and utilization management is preferredKnowledge, Skills and Abilities
- Knowledge of normal growth and development for infants to young adults
- Knowledge of family functioning and the nature of chronic illness/disability and its impact on the patient and family
- Skill in individual, family and group treatment modalities.
- Ability to effectively communicate both orally and in writing with interdisciplinary team members, family members, community agencies, referral sources and payers.
- Ability to work effectively with an economically and culturally diverse population.
- Ability to balance patient/family needs with constraints imposed by third-party payers and agency regulations.
- Knowledge of third party payer systems including Medicaid and Managed Care Organizations

REPORTING RELATIONSHIPS Supervision Received Reports to the Director of Care Management/Social Work/Admissions. The LMSW also receives clinical supervision from a LCSW-C.Supervision Exercised LSCW-C with 3 years experience at MWPH, may provide clinical supervision to Social Work Students observational experiences.TOOLS/EQUIPMENT USED Computer, telephone, fax machine, photocopier, on-line information system, personal pager PHYSICAL ENVIRONMENT/WORKING CONDITIONS Frequent stress due to multiple demands, shifting priorities, shifting populations and tight time frames. Risk of exposure to body fluids, infectious waste or blood borne disease.
