As a Clinical Staff Pharmacist you will: •Maintain excellent working knowledge of pharmacy practice and the Electronic Health Record. Verify and enter orders accurately, to ensure barcode scanning, timely accessibility of drugs, formulary maintenance, adherence to policy, and overall patient safety.•Engage all members of the healthcare team, including patients and families to design, recommend, and monitor patient-specific pharmacotherapy, participate in multidisciplinary rounds, provide patient counseling and other follow up; respond to code emergencies.•Accurately and safely prepare, label, check, and dispense all medication •Communicate with team members regarding patient and department needs directly and utilizing shift reports and daily huddles, including information for Pharmacy Leaders, Clinicians, Technicians, Procurement and 340B, Information Technology, and others.•Practice pharmacy stewardship through approved clinical protocols, medication dosing, therapeutic substitution, antibiotic stewardship, communicating with the healthcare team•Maintain and supervise a working knowledge of Pharmacy Technician workflow, duties and skill sets to ensure smooth department function and throughput•Participate in team meetings and others as assigned•Additional duties as assignedQualified candidates will possess:•Graduation from an ACPE accredited pharmacy school, with PharmD degree; alternatively, Bachelor of Science in Pharmacy with acute hospital/clinical experience, required.•Strongly Preferred: Minimum one year of hospital pharmacy practice•Considered: Recent graduate w/ minimum 2 years hospital pharmacy technician/intern experience; or non-hospital pharmacist experience, with counseling and medication therapy management•Supervisory/management experience preferred•Strongly Preferred: Minimum three years of hospital pharmacy practice experience; alternatively, completion of an ASHP accredited PGY-1 Residency, with minimum one year of practice experience preferred•Strongly Preferred: Experience responding to Code Blue calls; ACLS certification/training •Strongly Preferred: Performing Clinical Skills: medication dosing (vancomycin, aminoglycosides), renal dosing adjustment, antimicrobial stewardship, anticoagulant (warfarin, heparin, DOAC) dosing•Strongly Preferred: Specific experience with NICU, the Emergency Department, Critical Care/ICU•Strongly Preferred: Interpersonal skills and comfort with communications with nurses and physicians•Strongly Preferred: Knowledge and experience with IV compounding (USP 797 & 800), automation (Pyxis, carousel, etc)•Strongly Preferred: Experience supervising pharmacy techniciansWork Schedule:This position is an overnight position consisting of 10 hour shifts, 7 on and 7 off. The candidate will work with another pharmacist and two technicians.#GR8 Other
Pay Range:$46.18 - $69.27In cases where the range is displayed as a $0 amount or if the position is Per Diem (offering a fixed rate), salary discussions will occur during the screening process.United States of America (Non-Exempt)
