You help staff with the development and implementation of a realistic individualized plan of care for each patient, providing revisions when necessary. Whenever you receive calls from field staff and physicians regarding care orders or issues, you respond in a timely manner as well as ensure that orders are entered correctly. You also take care to update any medication profiles or schedules when you receive new information.In addition, you are responsible for holding weekly case conferences to address any decisions that have been made based on case findings. You understand the importance of referral source satisfaction and are always looking for new ways to improve patient outcomes and satisfaction. Upon receiving a referral from physicians or staff, you enter them into our Home Care Home Base accurately and in a timely manner. At times, you serve as the scheduler and help out as a staff nurse when needed. You help with interviewing, hiring, staff evaluations, and disciplinary actions alongside the Clinical Manager when it's expected of you.The well-being of your patients is always a top priority for you, which is why you diligently practice infection control and take universal precautions to ensure your patients are always safe. - Licensed as a registered nurse (RN) in the state of Oklahoma with good standing
- 2+ years of nursing experience in the home health care industry
- CPR certification OR the ability to obtain one within 60 days of being hired
- Knowledge about Medicare guidelines regarding home health agencies
- Valid driver's license, reliable transportation, and proof of insurance
