JOB SUMMARY

The Clinical Support Specialist works under the supervision of the Practice / Program Manager to provide referral and patient flow support to NHFHS Care Teams.

DUTIES PERFORMED:

* Demonstrates a working knowledge of the EHR, clerical and referral processes.

* Responds to health care providers, patients and families in a courteous manner.

* Helps support the achievement of provider productivity goals.

* Establishes and maintains effective working relationships with all members of the health care team and demonstrates appropriate communication skills.

* Accepts patient-related information from various sources and ensures reports are immediately forwarded to the appropriate party.

* Works to process practitioner referral orders in a timely fashion and forwards necessary EHR information as needed and required for each individual order.

* Schedules In-House referrals as needed and follows established processes and guidelines.

* Schedules External referrals for various procedures including, but not limited to, radiological procedures and consultations with specialists as ordered by medical providers and directs patients accordingly.

* Obtains necessary referral pre-authorizations.

* Records for referral orders are forwarded via EHR Fax.

* Communicates all stat reports/issues immediately to appropriate team member (MD, FNP, RN / LPN or MOA).

* If unable to complete a referral, reports immediately to the referring practitioner.

* Understands the purpose of and has a working knowledge of Patient Centered Medical Home concepts.

* Tracking: works with Task Management, as needed, to track outcomes for referrals, hospitalizations, pap smears, vouchers and other required information supporting Patient Centered Medical Home guidelines.

* Communicates clerical supply needs to the immediate supervisor in a timely manner.

* Contributes to the provision of safe, high quality patient care and proper maintenance of the Medical Record.

* Contributes constructive ideas for a more efficient referrals process.

* Attends staff development programs as required.

* Attends monthly staff meetings / developmental programs and contributes constructive ideas for more efficient referral processes.

* Maintains confidentiality of patient information at all times.

* Must hold all patient Protected Health Information (PHI) other patient personal information and agency information in confidence, in accordance with the attached Employee Confidentiality Statement, which I have read, understood and signed.

* Actively participates in and complies with all aspects of the NHFHS Corporate Compliance Program, follow the Program Code of Conduct and obey all relevant laws, statutes, regulations and requirements applicable to Medicaid, Medicare and other State and Federal health care programs.

* Participates in CQI, other internal committees, special project/observances or activities that promote improvements in organizational performance and/or advance the mission, goals and objectives of New Horizon Family Health Services.

* Observes schedule for work, breaks and lunch.

* Performs other duties as assigned by the Supervising Charge Nurse, Nursing and Clinical Support Services Director, Chief Medical Officer, Chief Operating Officer and Chief Executive Officer.

QUALIFICATIONS

Education:

High School Diploma or GED equivalent.

CNA / MOA I, PCT or a minimum of 1 year of related work experience

Preferred:

1 - 2 Years Higher Education in related health or social work fields

Electronic Health Record Experience

Other Requirements:

* Must have working knowledge of computer programs and professional construction of emails

* Must demonstrate

* ability to speak clearly and in a grammatically correct manner

* Must demonstrate the ability to accept responsibility as indicated.

* Must be a motivated and enthusiastic learner.

* Must present a neat appearance and adherence to the Nursing Department dress code

* Follows NHFHS policies and procedures including personal visits and phone calls
