Description
As a Clinical Support Staff in the Youth ACT program, you will help children and families acquire the necessary skills to manage their health, improve family relationships, and develop opportunities for social activities and interactions. You will report to the Youth ACT Program Director and you will work onsite and in the community.
Responsibilities:
+ Help complete the intake process and admissions with identified clients / families in Youth ACT
+ Risk Assessment
+ Substance Abuse Screen/Assessment
+ Suicide Assessment
+ Mental Health needs assessment
+ Social Needs Screening
+ Comprehensive Psychosocial Assessment to include diagnosis and needs to develop treatment plan (IAP)
+ Obtain release of information forms for all current past mental health providers, inpatient and outpatient, school, primary care provider
+ Ensure compliance with all agency/state/federal requirements for completion of client paperwork
+ Provide support to achieve daily tasks related to identified treatment goals
+ Facilitate/Coordinate tele-mental health services
+ Coordinate and assist team with job development by establishing educational, vocational and professional goals
+ Facilitate implementation of skills necessary to accomplish treatment plan goals and adjust to work towards established treatment goals
+ May have the opportunity to supervise student interns
Requirements and Education:
+ Requires a Bachelor's degree; Masters degree preferred.
+ Must have a valid drivers license.
