Our unique workplace Culture of Excellence is built upon:Employee engagement, empowerment and growthTeamwork toward our common goal - providing exceptional health care services with a passion for making lives betterAn atmosphere of caring and quality that cascades throughout the organizationThis position provides direct service to patients in a Behavioral Health Program area.Responsibilities:ESSENTIAL DUTIES AND RESPONSIBILITIESCompletes psychosocial assessments and treatment plans in cooperation with patient and multidisciplinary team.Conducts a comprehensive assessment of the clinical needs of each patient prior to beginning individual, family or group therapy.Works collaboratively with the patient to develop a treatment plan addressing patient's identified treatment goals.Delivers behavioral health care in accordance with patient evaluation, treatment plan and within the scope of their license.Works directly in treatment relationship with patients, individually or in groups, in consultation with Psychiatrist/NP.Provides crisis assessment and intervention.Consults and coordinates care of patient with physician/NP, therapists and community agencies.Provides appropriate discharge planning.Refers patient or patient's family to other community resources.Participates in clinical collaboration and/or psychiatry supervision to discuss cases and determine appropriate course of treatment.Maintains pertinent patient data and documentation in medical record in accordance with state, TJC and Mercyhealth system guidelines.Attends required meetings or staffing as indicated by Program Manager.Participates in performance improvement activities.Performs other duties as assigned.Complies with all Mercy Health System, State and JCAHO policies and regulations.EDUCATION & EXPERIENCEMaster's Degree in Counseling, Social Work, Psychology or other related field and appropriate level of field experience and coursework.CERTIFICATION/LICENSURELicensed to independently provide psychotherapy in Wisconsin (LCSW, LPC, LMFT)TACT Certified Level II (as assigned by supervisor)CPR Certified (as assigned by supervisor)ADDITIONAL REQUIREMENTSPassing the Driver's License Check and/or Credit Check (for those positions requiring).Passing the WI Caregiver Background Check and/or IL Health Care Workers Background Check.Must be able to follow written/oral instructions.OTHER SKILLS AND ABILITIESWORK ENVIRONMENTThe noise level in the work environment is usually quietOccupational Exposure: Category C - No partners in the specified job classification have occupational exposure.AGE OF PATIENTS SERVED (Please remove what is not applicable)Pediatric (1 year - 12 years)Adolescents (13 years - 17 years)Adult (18 years - 64 years)Geriatric (65 years and older)INFORMATION ACCESSPartner may access patient care information needed to perform their job duties.WORK CONTACT GROUPPatients, families, physicians, community and regulatory agencies, and other hospital and clinic personnel.SPECIAL PHYSICAL DEMANDSThe Special Physical Demands are considered Essential Job Functions of the position with or without reasonable accommodations.
