Job description
The Counselor is responsible for providing professional therapy to patients in order to prepare the patient's for transitioning to the next level of care in both an individual and a group setting.
Duties/Responsibilities:
· Conducts initial assessment of all new patients; interviews and evaluates patients.
· Identifies factors which may interfere with obtaining maximum benefits from treatment.
· Develops and implements patient treatment plans, including the coordination of referrals to specialty and social services, the recording of progress notes and the provision of continuity of care.
· Participates in monitoring and evaluating the quality and appropriateness of patient care.
· Provides appropriate therapeutic services to assist patients and families in coping with presenting symptoms and problems.
· Participates on interdisciplinary treatment team.
· Provides professional therapy to patients in order to maintain or restore highest level of functioning.
· Provides general or specialized substance abuse counseling services as needed
· Works in conjunction with Clinical Supervisor to create therapeutic intervention and philosophy for caseload
· Performs patient record review to make sure records are compliant Pennsylvania state and federal regulations
· Maintains caseload of patient and responsible for case managing cases and collaborating with other providers to provide quality patient care
· Performs individual and group counseling under the supervision of Clinical Supervisor as needed.
· Maintains records and makes written and oral reports concerning patient care, counseling and therapeutic activities.
· Prepares reports and other documents concerning patients.
· Complies with company and department policies and procedures, quality assurance, safety, environmental, infection control and complies with requirements of accreditation and regulatory agencies.
· Conducts specialized in-service training in area of expertise for various audiences.
· Attends and participates in staff development programs, department related conferences and administrative meetings.
· Establishes and maintains appropriate network of professional contacts.
· Maintains up to date with professional organizations and publications.
· Attends and participates in meetings, conferences, etc.
· Represents the company and/or unit as assigned.
· Perform other related duties as assigned or requested
Education and Experience:
· At least 2 years of experience in the field of mental health/substance abuse/rehabilitation
· Experience in providing professional social work services to patients and their families.
Job Type: Full-time
Pay: Up to $85,000.00 per year
