Overview:
Job Profile Summary Consults and coordinates care of patient with physician, program coordinator and agencies. Assesses treatment planning and progress. Prepares treatment plan and goals for each individual patient/resident. Provides crisis, assessments and intervention. Maintains pertinent patient data in medical record in accordance with state and guidelines. Offers in-service education programs and functions as a resource to other staff and the community as needed. Participates in development and selection of patient education materials and services. Provides individual, family, and group therapy for residents and assists other staff with group materials. Performs other duties as assigned. Education and Experience:Master's degree in Behavioral Health field and appropriate level of field experience and coursework.
Certification/Licensure:Licensed in one of the three state licenses (LMFT, LCSW, LPC) or a higher designation.
