Location New York, New York Shift: Day (United States of America) Description: Clinical Women's Health- Behavioral Health Care Specialist -SW - Weill Cornell The Social Worker will work daily with HT5-WHP leadership and serve as a liaison between the practice and designated MAC-IMP programs geared towards behavioral health support and coordination with Pediatrics for enrolled patients. The Social Worker will ensure efficiency in providing direct provision of services, while working with the team to implement programmatic goals. The Social Worker's primary function will be to provide behavioral health support and coordination to practice patients and work in partnership with Social Work team to perform psychosocial assessments, support and referral when needed to practice patients. The Social Worker assesses and identifies psychosocial needs and develops criteria to assure that patients and families in need of social support services obtain assistance. The Social Worker is responsible for duties which include, but are not limited to: Engages patients/families in a professional treatment relationship to assess the need for social work intervention; uses methods consistent with sound social work practice in order to help the patient/family cope with psychosocial problems Individual interviews with patients and relatives
  • Participate in regular meetings with HT5 and MAC-IMP teams
  • Serve as primary liaison for Perinatal Mental Wellness program
    • Manage patient list and provide additional social services support to patients with positive depression screening.
  • Provide behavioral health support to patients:
    • Declined or not eligible for Perinatal Wellness program.
    • Patients who have transitioned from wellness program up to one year following birth with plan to transition to long term support
    • Develop treatment plan that includes identification of community and ongoing support services.
  • May provide supervision to staff and social work students to teach the skills of the profession and provide knowledge of hospital policies and procedures
The role is onsite, Monday - Friday, 9:00 AM - 5:00 PM Preferred Criteria
  • Licensure in Clinical Social Work (LCSW)
  • 1-3 years post master's experience preferred
  • Experience working in a hospital and/or healthcare setting preferred.
  • Experience working with a Maternal and Infant population preferred
Required Criteria
  • Master's Degree in Social Work (MSW)
  • New York State License as a Licensed Masters Social Worker (LMSW)
  • Experience providing evidence-based psychotherapeutic approaches; cognitive-behavioral therapy experience.
  • Ability to thrive within a complex medical system and working understanding of the healthcare system.
  • Must have training and experience working with children, adolescents and families with mental health concerns.
