Job Description:
Title: Clinician Department: Greater New Haven Health Care for the Homeless Reports To: Clinical Director of Homeless Care Responsible for both Street Outreach with the Healthcare for the Homeless program and In-Office clinical services. Street Outreach will consist of relationship building with clients utilizing evidence-based practices and harm reduction to engage and support their needs. In-office clinical services will provide one on one care under the support and guidance of the Clinical Directors of the Recovery and Wellness Center. In-Office care will take place in the medical van or designated office site. The level of care provided will be consistent with the level of licensure and education. Essential Duties and Responsibilities:
  • Provides professional clinical services to individual clients, groups of clients, and/or families in the CS-HHC setting or general community
  • Maintains client confidentiality at all times and ensures to follow agency, state and HIPPA laws
  • May perform oversight activities with Clinician I's as directed by the Program Director or Assistant Program Director
  • Establishes and maintains good working relationships with community agencies
  • Conducts street canvassing and outreach within the New Haven Region to offer unsheltered individuals linkages to services and to the Coordinated Access Network (CAN)
  • Works as a team in support of the medical providers when assigned
  • Will perform outreach during non-traditional hours to include early, late and weekend hours as needed
  • Complete verification of homelessness and assist with completing verification of disability from the providers in support of the CAN Entry and Exit process
  • Evaluates data relating to the bio-psychosocial consequences of physical and emotional illness, developmental disability and/or substance use
  • Implements standardized evaluations or assessments in conjunction with multidisciplinary team as appropriate
  • Formulates bio-psychosocial assessments and diagnosis, treatment plans/reviews and goals according to a site-specific timeline
  • Completes documentation in Epic, DDaP and HMIS as required by grant and medical standards
  • Attends and participates in team conferences (if applicable)
  • Ensures required documentation is complete, timely and accurate at all times
  • Obtains and tracks proper insurance authorizations as needed to provide services
  • Serves as a behavioral health resource to students, teachers and parents (if applicable)
  • Actively monitors client risk factors and takes action to ensure client's safety while following CS- HHC policies
  • Attend supervision meetings as required
  • Performs all other duties as required
Qualifications:Education: Master's Degree in a Behavioral Health field required. Licensed or licensed eligible preferred (LMSW/LCSW, LPC-A/LPC, MFT/LMFT, LADC, PsyD, Ph.D) are encouraged to apply. Experience: Minimum of one (1) year experience working with relevant population required (internship may qualify as experience). Knowledge/Abilities: Knowledge and competency regarding diagnosis, assessment and treatment of behavioral health services. Excellent communication skills; both written and verbal. Ability to work with an interdisciplinary team. Ability to generate high levels of client satisfaction. Must demonstrate age specific/population specific competencies in area(s) of practice. Knowledge and skills related to individual, group and family system dynamics. Must demonstrate cultural competence. Benefits:
