The goals of the program are to prevent out-of-home placement for children with multiple stressors, provide a safe environment to return children already in placement, and prevent future out-of-home placement. You will report to the Program Coordinator and work within a treatment team, including an Intensive Case Manager and/or a Community Support Worker depending upon the needs of each family. You will work a hybrid schedule with a combination of onsite and remote with travel to clients' homes required.The work schedule is Monday through Friday with flexible scheduling that includes early evenings, to meet the needs of our client families, and a rotating phone on-call schedule.This is a great opportunity to learn and implement evidence-based practices in trauma- informed care; parent education; child and family well- being; and juvenile justice!Responsibilities:
- Provide home and community-based assessment, treatment planning, and clinical services with children, caregivers, and families (the level of intensity / number of direct services hours per family is dependent upon the program/ services in which one is employed)
- Provide individual, couples, and family counseling in the home, community, and agency offices as most appropriate and supportive to the family and their treatment goals
- Document services provided, treatment progress, and recommendations following agency, county, and state policy and funding requirements
- Provide liaison contact with collateral resources such as referring agencies, schools, family court, and other agency personnel serving on behalf of a child and family as it relates to their treatment
- Responsible for the clinical decisions when assigned and paired to work as a team with an Intensive Case Manager and/or Community Support Worker
- Ensure provision and coordination of regular treatment plan meetings and services plan reviews
Requirements and Education:
- Requires a Masters degree in Social Work, Mental Health Counseling, or related field
- Preferred minimum of one year of post-graduate experience
- Must have a valid drivers license
