Job Title:Care Coordinator II
Division/Program:Children's Behavioral Health Services Team (FSP)
Starting Compensation:70000 - 80000 USD Per Year
Working Location:Community-based in Stanislaus County
Working Hours/Shift:Monday - Friday 8 am to 5 pm

What you bring to SBHG:- Master's degree in child counseling, development or psychology, social services or vocational counseling or related social science field required
- Licensed as AMFT, ACSW, or APCC required.
- Valid California Driver's License required
- Bilingual in Spanish a plus
How you will make a difference:- Conducts initial and ongoing assessments of the youth and family to identify strengths and needs, to include diagnosing clients with emotional disturbances.
- Uses a whatever it takes approach to provide services in various settings, including homes, schools, community, and office settings. Uses personal vehicle to drive to and from offsite locations to provide services.
- Provides direct care services, including individual, group, and family therapy. Also provides case management and collateral services to clients and their families, as deemed necessary and appropriate through the assessment/plan of care process and in collaboration with the Child and Family team.
- Provides crisis intervention to youth and families as needed. Part of an after-hours crisis support on-call rotation.
- Documents all services provided in a clear and concise manner within the Electronic Health Record within the timelines and framework established by Central Star
