Job Title: Certified Nursing Assistant * PRN/Part-Time. * Schedule to be discussed at interview. Location: 915 Center Ave. Fort Collins, Colorado. About the Role: As a CNA at Columbine Poudre Home Care, you will: * Deliver personalized care to clients in their homes, ensuring their physical, mental, and psychosocial well-being as per individualized care plans. * Assist with daily activities such as grooming, hygiene, feeding, toileting, and mobility, while maintaining clients' dignity and comfort. * Support the ability of home health care clients to continue to live safely in their own homes. * Observe and report changes in residents' conditions or behaviors to nursing staff. * Maintain residents' skin integrity through proper care techniques. * Accurately document care provided and adhere to safety and infection control procedures. * Provide restorative care to help residents maintain or regain independence. * Participate in care planning meetings. * Perform other duties as assigned. What We're Looking For: Experience and Education: * Minimum 18 years of age. * High school diploma or GED preferred. * A Certified Nursing Assistant License in the state of Colorado. * 3 months of CNA experience required. Skills: * Ability to effectively able to read and interpret documents. * Ability to communicate effectively. * Basic mathematical skills. * Ability to reason and problem solve. * Basic technical skills. * Knowledge of the aging process, dementia, and disabilities. Our Total Rewards package includes a salary range of between $19 - $22/hr. depending on experience. We also offer a travel stipend and evening shift differential!
