Provide daily hygiene measures including bathing, oral hygiene, skin care, hair and nail care, as assigned and as required.Change bed linens as required.Dress and undress residents as needed.Serves and collects food trays.Assist in feeding and providing oral fluids to residents.Assist Residents in Preventive, Therapeutic, and Restorative Nursing ProceduresTake and record vital signs and weights.Turns and repositions residents who require assistance.Transports residents using wheelchair, walkers, lifts and other assistive devices.Perform range of joint motion exercises daily as needed.Assist residents to ambulate, or use assistive devices.Assist the resident in bowel and bladder elimination activities and retraining programs.Adhere To Safety and Infection Control Policies and ProceduresMaintain good personal hygiene and adhere to uniform requirement.Practice standard precautions.Perform cleaning of resident bedside unit, care equipment, and utility roomProvides for the safety of the resident using safety devices identified on the assignment sheet.Report personal illness or skin lesions.Participates in orientation training and twelve (12) hours of in-service programs annually.Report defective equipment or other potential safety hazards promptly.Adheres to Nursing Department policies and procedures.Communicate Effectively Orally and In WritingReport physical or behavioral changes in the resident, accidents/incidents immediately.Document resident nutrition and fluid intake and outputs.Document daily on facility forms and in resident's medical record.Clarify questions regarding assignment with charge nurse.Report all grievances and complaints appropriately.Develop solutions using problem solving skills.Control angry feelings.Maintain and safeguard entrusted confidential information.Be flexible, receptive and adaptive to change.Exhibit teamwork with all employees and hold others accountable in delivering the best care.Promote Positive Public Relations for the FacilityDisplay courteous, cooperative, helpful manner with residents, visitors and other staff.Demonstrates self respect and respect for others.Reports to work when scheduled and on time. Use proper telephone etiquette.Represent the facility in a positive manner. Perform Other Tasks as RequiredBe able to complete documentation in the electronic medical record.Assist other personnel when assignment completed.Perform all tasks in accordance with established policies and procedures and in a timely manner.Knows and follows existing lines of communication and authority.Assures that resident rights and family requests are maintained at all times, reporting suspected deviations to an immediate supervisor.Begin and follow through with assignments without constant oversight. Participate in scheduled training, in-services, Relias Modules, and educational classes.Foster the mission, values, and guiding behaviors of the Sisters of Providence.Other - as designated by Charge Nurse or Director of Nursing.RequirementsQUALIFICATIONSAbility to read/write comprehensive English and document observations. Must perform basic math. Ability to verbally communicate with residents, staff and general public in a manner that is understandable, must be honest, dependable and exhibit a warm, cheerful, caring manner. Must be able to safely perform the essential functions of the job with or without reasonable accommodation.EDUCATION and/or EXPERIENCECurrent CNA Certificate in good standing on file with the Indiana Nursing Assistant Registry. One year experience in long-term care or other health care setting desired.PHYSICAL DEMANDSPhysicalWill need to continuously stand and walk, squat, bend to floor, reach, twist, sit and potentially remain in uncomfortable positions for extended periods of time. Must be able to lift, move and turn residents. Requires varying degrees of pushing, pulling and lifting depending on the weight of the resident, which is often times over 100 pounds. Must have eye/hand coordination sufficient to assist residents with ADL's, transfers, and ambulation. Must be able to clearly speak. Must deal calmly with multiple tasks, noises, and interruptions on a continuous basis.SensoryMust be able to hear normal sounds and voice patterns with constant background noise. Must be able to receive and follow verbal instructions. Must be able to respond to audible emergency signals, alarms, call lights and telephones. Must be able to smell smoke, spoiled food, incontinent residents, etc. CognitiveMust be able to remember multiple assignments over an extended period. Must be able to remember specific requests, ideas and concepts and relate them as necessary. Must be able to concentrate on details during constant interruptions.
