POSITION SUMMARY:The Hospice CNA is responsible for providing personal care under the direction of a registered nurse. Performs all assignments with honesty, integrity, and care that promotes the mission, vision, and values of Friendship Village.KEY RESPONSIBILITIES AND ESSENTIAL FUNCTIONS:•Performs personal care (bathing, dressing, toileting, feeding, etc.) according to the Hospice Aide Plan of Care.•Documents care provided according to agency policy. •Reports any deviations from the Plan of Care to the Case Manager or designee.•Uses Durable Medical Equipment safely.•Takes vital signs when on Hospice Aide Plan of Care.•Fosters cooperative effort among personnel by understanding other people's functions in patient care and by actively participating in team and staff conferences.•Provides proper care and observation of the patient's skin to prevent the breakdown of tissue over bony prominence. Reports any changes/areas of concern to the Case Manager.•Assists with ambulation, transfers, and range of motion if documented on the Hospice Aide Plan of Care.•Provides respite for patient's family/caregiver when on-site, as appropriate.•Participates in QAPI activities.•Participates in IDG meetings, as appropriate.•Contributes to continuity of care by effectively communicating services provided and changes in the resident's condition or family interactions in a timely manner.•Complies with FV infection prevention and control policies and procedures. •Demonstrates procedural competencies for assigned tasks.•Complies with all Friendship Village policies and procedures.•May perform other duties as assigned or requested.
- Must be a licensed driver with an insured automobile in good working order.
- Successful completion of a state-approved geriatric nursing assistant training program. Registration with the State registry is required.
- CPR certification is mandatory.
