POSITION SUMMARY:The Certified Nursing Assistant (CNA) in the Ready for Discharge Unit (RDU) provides essential, supportive care to patients who are medically ready for discharge but are experiencing barriers. The CNA works under the supervision of Registered Nurses (RNs) and Licensed Practical Nurses (LPNs), performing duties authorized by the Oregon State Board of Nursing (OSBN) and Sky lakes Medical Center. This role requires a commitment to compassionate, efficient care to support the patient's health, safety, and comfort during their transitional period in the RDU.QUALIFICATIONS:Required:Current Oregon CNA certification; proficient in performing authorized CNA tasks, including ADLs and vital signs monitoring; strong communication skills and the ability to work as part of an interdisciplinary team; attention to detail and ability to document care accurately and in a timely manner; empathy, patience, and a strong commitment to providing compassionate care.Lic/Reg/Cert:Current Oregon CNA certification (CNA II, Acute Care, certification preferred).TYPICAL PHYSICAL/MENTAL DEMANDSMust be in good general health, able to frequently stoop, bend, reach, stand, walk, and sit. Must be able to lift at least 50 pounds or more and push/pull hospital equipment. Must be able to function during periods of stress, trauma and/or diseaseESSENTIAL JOB FUNCTIONSPatient Care and Support:
- Assist patients with activities of daily living (ADLs), including but not limited to bathing, dressing, toileting, grooming, feeding, mobility, and repositioning as assigned by RN or LPN and as appropriate for age, developmental level, and condition.
- Measure and record patient vital signs, intake, and output; report significant findings to the RN or LPN immediately.
- Monitor patients for changes in physical, emotional, or behavioral status, and report observations promptly.
- Assist in discharge readiness activities as directed by the RN or LPN.
- Accurately document all care provided in the patient's electronic health record (EHR) as per policy.
Communication, Collaboration:
- Collaborate effectively with RNs, LPNs, and other members of the healthcare team to ensure continuity of care.
- Communicate patient care information promptly and accurately to supervising RNs or LPNs.
- Demonstrate professionalism during all interactions with patients, co-workers, patients, family members and visitors.
Quality Improvement, Safety, and Compliance:
- Adhere to all hospital policies, procedures, and protocols.
- Maintain a clean, safe, and organized patient care environment
- Participate in infection prevention and control measures, including proper hand hygiene and personal protective equipment (PPE) use.
