Job Type Full-timeDescriptionThe Boys & Girls Clubs of Greater Tarrant County is seeking a highly motivated and compassionate Case Manager to join our Comin' Up team. In this position you will develop and implement individualized plans that address the unique needs of each member. Services are provided on site during program hours (Monday-Friday, 7:00pm to 11:00pm) and at times, may be required to provide support services virtually and assist with community outreach. Position requires working with at risk and gang involved youth.The Comin' Up Gang Intervention Program serves at-risk and gang-involved youth between the ages of 12-18 years in Fort Worth communities. In partnership with the City of Fort Worth, the program provides needs-based services and activities in an effort to reduce gang violence and influence positive life changes for our youth.Essential Functions and Responsibilities: Case Management:
- Provide holistic and comprehensive case management services to members at assigned site(s) including but not limited to: intake, assessment, goal setting, service planning, progress monitoring, follow up services, advocacy, documentation, and referral services
Participation:
- Accountable for promoting site participation
Comin' Up Programming:
- Accountable for assigned programming and activities
- Accountable for quality programming and activities
- Actively participate on multi-disciplinary team
- Document all services and contacts
- Accountable for accurate and timely case notes, data entry and report
Administrative Compliance:
- Complete all required reports and administrative duties on time.
- Collect and manage required data and internal and external reporting. Ensure accuracy of data and reporting. Ensure the effective completion of all assessment and measurement activities of programs
- Accountable for accurate and timely collection and review of membership application
- Comply with all administrative policies and procedures, processes and controls
- Accountable for accurate and timely paperwork, reports, and data entry
- Accountable for utilizing all BGCGTC communication tools regularly and effectively
Facility and Child Safety:
- Accountable for adherence to all organizational health and safety policies and implementation of all safety practices and procedures at assigned work location(s)
- Take all necessary and reasonable precautions to protect members, staff, equipment, materials, and facilities. Abide by facility maintenance protocols and procedures to ensure compliance
- Accountable for reporting incidents and completing documentation within required timeline
- Maintain discipline, de-escalate conflict between members and/or others
- Obtain and renew CPR and First Aid certification by deadline
Relationships:
- Be a positive role model
- Maintain emotional control and make good decisions under pressure
- Represent the Boys & Girls Club in a positive manner
Attendance:
- Regular attendance is required; arrive on time, work required hours/shift, and leave at scheduled time
RequirementsEducation and work experience requirements:
- High School diploma or equivalent.
- Minimum of 1 year of experience working with at risk youth and/or gang involved youth.
- Minimum of 6 months experience with case management.
Minimum Qualifications:
- Ability to work regular program hours which includes day, evening and night hours with periodic weekends (required evening programming hours are 7:00pm - 11:00pm Monday - Friday; required day time hours will vary based on program need)
- Ability to assist youth to progress and achieve their individual goals
- Ability to establish rapport and build relationships
- Ability to identify needs and find solutions
- Ability to work with at-risk youth
- Knowledge of case management techniques and/or processes
- Knowledge of conflict resolution or mediation techniques
- Knowledge of technology and software such as tablets, social media, cloud based tools, virtual meeting tools
- Must be able to obtain CPR and First Aid certification
- Must be able to pass any and all required background screens and drug screens at any time
- Position requires driving personal vehicle and company vehicle. Candidate must have satisfactory driving record, valid driver's license, reliable transportation, and liability insurance for operation of a motor vehicle relevant to carrying out duties and to be covered under company insurance the requirement is for the employee to be a minimum of age 21
Preferred Qualifications:
- Bilingual
- Bachelor's Degree from an accredited college or university or equivalent experience
- Licensed social worker or counselor
- Work experience or program knowledge of the Boys & Girls Clubs Movement (national and/or local Clubs)
Pay & Benefits:
- Minimum starting pay: $19.51 per hour
