Work Shifts 9:00 A.M - 5:00 P.M Job Description Purpose of Position: Accountable to the Coordinating Manager B of Social Work, the Community Liaison Worker is responsible encompasses clerical, liaison and follow-up operations for the psychiatric in-patients who are being referred to SNFs, RTC, and/or transfe1Ted to other hospitals. The program covers adults, children and adolescents for 30 days post discharge. Responsibilities: •Assists in the coordination of psychiatric out-patient appointments for all patient residing outside of the hospital's catchment area. •Under the supervision of the unit social worker, helps patients complete referral for entitlements and other community resources needed for discharge •Collects data completed by ward staff on a daily basis concerning patients being discharged in that day •Enters individual patient data into the computerized Psychiatric Discharge Management System (PDMS) •Meets with patients prior to discharge to explain the Koskinas Program and obtain agreement for outreach services •Utilizing the PDMS system, tracks and inputs housing and treatment outcomes for all patients eligible and in agreement with Koskinas follow-up •Calls patients, community agencies and treatment resources within 72 hours of discharge to determine successful compliance with discharge plans. •Sends reminder letters, makes reminder calls and offer supportive interventions to expedite patient follow-up with housing and aftercare plans. •Consults with the Coordinating Manager of Psychiatric Social Work if patients in the community are deemed at risk or require further intervention •Collects accumulated discharge data on the PDMS system and submits to HHC on a prescribed basis as part of the hospital's compliance with the Koskinas Program. Minimum Qualifications For Appointment to Level I 1. One year of full-time experience in a government agency or community organization in providing community services to the public or in assisting members of the community in obtaining community services; or 2. Completion of an approved six month training program of combined classroom training and on-the-job experience in community liaison work, plus six months of full-time experience as described in 1 above. For Appointment to Level II In addition to meeting the qualification requirements of Assignment Level I, appointment to Level II requires: 1. Two additional years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or 2. Education and/or experience equivalent to 1 above. Study at an accredited college in sociology, psychology or other behavioral science may be substituted on the basis of 30 semester credits for each year of the experience described above.
