COMMUNITY BEHAVIORAL HEALTH SPECIALIST

Job Overview:
The Community Behavioral Health Specialist provides case management services to assigned caseload. Services are provided to maximize an individuals immediate and continued community functioning while achieving and sustaining recovery/resiliency from mental illness and/or substance us disorders. The Community Behavioral Health Specialist completes initial and annual assessments and develops wellness plans in accordance with clients individual treatment goals. This position conducts education groups as assigned and is part of a multidisciplinary team.
What you'll get to do:
+ Complete initial and annual assessments and other required documentation per departmental guidelines, within the electronic medical record (EMR) for each individual on the assigned caseload. Complete any additional assessments that are assigned by the team leader or clinical supervisor
+ Develop person-centered and strengths-based wellness plans in coordination with the completed assessments.
+ Conduct risk assessments as part of the assessment bundle and develops supportive care plans as a preventative risk measure.
+ Provide educational information and skill building in identified areas of concern to assigned client/family on topics such as daily living skills, education, symptom management, communication/relationships with a focus on improving functioning and quality of life.
+ Act as liaison to community and service organizations/programs including but not limited to Public Administrator/guardians, employment or educational entities, social service providers, legal/financial agencies as well as access to necessary wellness and rehabilitative services.
+ Assist assigned clients in gaining access to resources, both internal and external which support recovery goals.
+ Facilitate psycho-educational groups for clients enrolled in ACPR services.
+ Serve on departmental, program, or agency committee(s) or task team(s) as assigned.
Qualifications:
+ Masters in Counseling, Social Work required
+ Licensed or provisionally licensed preferred
+ Minimum two years working with adult SPMI or children with SED preferred.
