The Community Care Coordinator - Health Services at UnitedHealthcare is responsible for coordinating and implementing individualized care plans for long-term care needs. This role involves assessing, planning, and implementing care strategies, as well as advocating for individuals and families to ensure their needs are fully represented and supported by the health care team. The position also requires conducting home visits, initiating referrals for social service programs, and serving as a resource for community care coordinators. This is a field-based position with a focus on the Lyon, Miami, Osage, and Franklin counties in East/Central KS.
The ideal candidate will have a Bachelor's Degree in a related human services area, 1+ years of experience in community health care, and the ability to travel up to 75% of the time for the role. Preferred qualifications include a clinical degree and background in managing populations with complex medical or behavioral needs.
