Job Description:
The Community Education Specialist is responsible for providing a System of Care approach to do the following: identify training needs based on gap reviews; design member-facing educational curriculum; coordinate and organize member and stakeholder trainings; and develop member educational materials related to Tailored Plan and Medicaid Direct operations. This position seeks to provide appropriate and timely information and to teach and inspire change within public and private entities that routinely interact with individuals with health needs or other high-risk populations. All Education efforts are designed to improve access to care, encourage prevention, advance health directed care, promote community-based and system-wide approaches, processes, and programs. This position works closely with all Community Health and Well-Being team members to ensure implementation of the member and stakeholder educational curricula. Based on high levels of collaboration, the Community Education Specialist focuses on system-level solutions to increase timely access to needed services and identify training gaps and trends.This position will allow the successful candidate to work fully remote from their home office or can work hybrid from the Alliance office assigned, if preferred. The candidate must reside in North Carolina and be available for travel in the Alliance catchment area.Responsibilities & DutiesPublic Education
  • Implement, evaluate, and execute the Community Education Plan focusing on behavioral and physical health, wellness, prevention education, chronic disease, preventive care, disease management health screenings, and secondary and tertiary prevention utilizing evidence-supported training
  • Provide information, education, and consultation to public and private entities that routinely interact with individuals with special health care needs or other high-risk populations
  • Develop, implement, and evaluate Alliance education and training activities resulting in increased and improved access to public healthcare and Systems of Care approaches
  • Collect information associated with the results of the education events and reviews the impact of each initiative, making changes and appropriate adjustments based on that information
  • Oversee Alliance initiatives that aim to support high risk individuals and individuals with special health care needs that are enrolled in the Alliance care management
  • Make referrals and aids in the early identification of individuals with special health care needs
Collaborate with internal and external stakeholders
  • Partner and collaborate with community partners on support, education, advocacy, and research using a Recovery, Trauma Informed, and System of Care framework. Use marketing and promoting approaches, projects, and trainings that result in active collaboration and partnership with key stakeholders
  • Engage the community in supporting and collaborating to further the mission, vision, and values of Alliance and the Community Health and Well-Being department through forming partnerships and relationships with key stakeholders and Community Based Organizations (CBO)
  • Interact with the community in varying capacities such as target-specific marketing, social media engagement, public awareness, and advocacy
  • Serve as a resource to the community, through education and response coordination, for healthcare, and crisis and emergency management
Outreach
  • Assist with identifying opportunities for education and outreach, such as: the target population, the best location, and strategies
  • Develop resources that can be utilized by enrollees of Alliance
Minimum RequirementsEducation & ExperienceBachelor's degree in public health, health education, or related human services from an appropriately accredited institution and (3) years of experience. Reliable transportation is required for business. Must be able to transport self and materials to various community sites with flexible days and hour.OrMaster's degree in a human services field from an appropriately accredited institution and two (2) years of experience preferred.Special RequirementsBilingual Spanish PreferredKnowledge, Skills, & Abilities
  • Demonstrate exceptional interpersonal skills, highly effective communication ability, and the propensity to make prompt independent decisions based upon relevant facts
  • Demonstrate problem solving and conflict resolution skills
  • Demonstrated knowledge of behavioral health and intellectual/developmental disabilities and extensive knowledge regarding community services
  • Ability to become proficient in all disability areas, including areas where co-occurring illnesses exist.
  • Demonstrated knowledge of the primary purpose and functions of community agencies.
Salary Range$28.12-$35.25 hourlyExact compensation will be determined based on the candidate's education, experience, external market data and consideration of internal equity. An excellent fringe benefit package accompanies the salary, which includes:
