Job Description:

Performs life skills training and supervises individuals who live independently in the community or with their families. Provides ongoing support and assistance to the participants by aiding in their self-care. Assists them in achieving all of their habilitative goals.

*Staff assigned to the CHP II role provide community hab services to behaviorally complex individuals. This position requires ongoing review proficiency in individual plan care documentation including, but not limited to, IPOPS and BSP where applicable. CHP II staff may be required to maintain ongoing certification in various medical and/or behavioral training modules including (but not limited to) SCIP-R and CPR provided by AHRC based on individual needs.

1. Assists individuals in achieving all of their goals, as found in their Community Habilitation goal plan.

2. Ensures all activities of daily living are supported and performed. This includes, but is not limited to, personal hygiene tasks such as, tooth brushing, light grooming, assistance in the restroom (adult diaper care/menses care if applicable). This also includes responsibilities such as assisting with dressing, meals preparation (making healthy choices), socialization skills, community safety skills, shopping skills, money skills, housekeeping and laundry skills.

3. Provides companionship, supervision, role modeling, and instruction while ensuring freedom from any hazards in both the home and community settings.

4. Documents implementation of services and progress (or lack of progress) by the individual in the Medisked system within 24 hours of service provision.

5. Supports individuals in becoming as independent as possible in both their homes and in their immediate community.

6. Alerts the Program Manager of all situations, crises, or incidents that may affect the individual.

7. Contact the Program Manager concerning any change in schedule.

8. Performs all other job-related duties as requested by supervisor.Ensure that work-related safety procedures are in place and are being adhered to by staff. Observance of an unsafe act or a potentially hazardous work environment must be immediately reported to supervisor and/or safety committee.

High School diploma or equivalent preferred. Prior experience working with the developmentally disabled preferred. Must possess a valid and qualified NYS drivers license and maintain throughout employment. Must have access to an automobile to use when providing services, ability to communicate and understand written, oral, and email directives/information. Must have access to the internet to complete required service documentation.

This position may require ongoing certification and proficiency in various medical and/or behavioral training modules including (but not limited to) SCIP-R and CPR provided by AHRC based on individual need.

* This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services and, therefore, is subject to successful completion of fingerprint/criminal background check.
