We're thrilled to announce an exciting career opportunity to join the HKS team as a Community Habilitation Worker/DSP in our OPWDD Habilitation program in Nassau County.

This position offers both part-time and full-time opportunities with flexible hours.

The Goal of the Community Habilitation Worker is to provide assistance and support to individuals within their local communities. Services must either start or end within the individual's home. The Community Habilitation Worker must assist in the preparation and performance of support services outlined within the Community Habilitation Plan.

Essential Duties and ResponsibilitiesLearn and implement the Staff Action Plan for each participant (this could include ISP/Life Plan/SAP). This includes learning and understanding how to meet their mobility, dietary, medical, and behavioral needs.

  • Record data, daily and monthly notes, and any other billing documents accurately, and completely, and submit such records promptly. These are done electronically through Therap.
  • Work independently with limited supervision.
  • Participate in any program meetings/trainings and events as requested by your supervisor.
  • Assist in planning for and organizing Individuals Person Centered Plan and schedule for the consumer.
  • Accompany and assist individuals within their home and local community with all task-specific assignments outlined within the individual's Staff Action Plan and Life Plan.
  • Report to work in a timely fashion and promptly notify the Program Director and family of any absences or anticipated absences
  • Carry out assignments and/or activities as assigned by the Program Director.
  • Assist with transportation in the local community.
  • Must be flexible and able to work nights and weekends if needed.

Education and/or Experience

  • Minimum High School Diploma or equivalency required.
  • Experience working with the blind and/or developmentally disabled and knowledge of OPWDD regulations preferred.

PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by a Community Habilitation Worker to successfully perform the essential functions of this job.

  • Physically assist to lift individuals weighing 40 - 100 pounds or more
  • Reach above head and below knees
  • Endure repetitive motions with a right and left hand Reach out to pull and push a wheelchair
  • Make twisting and bending motions at the waist
  • Have a good reaction response to protect personal space

License/Certification:

Valid NY Driver's License Required

Experience working with the developmentally disabled.

Knowledge of OPWDD regulations prefer
