Description

BACKGROUND

Fifty years ago, four Black American mothers, aided by 25 volunteer Black physicians, launched Baywell Health, formally known as West Oakland Health amid the Civil Rights and Black Power movements. It immediately became an anchor for the Black communities in West Oakland, East Oakland, and Berkeley for healthcare, health education, food and housing programs, and voter registration. The clinic became a spark that transformed healthcare advocacy in California and the nation.

Baywell Health's purpose is to be the trusted hub advancing the Bay Area Black community's health and dignity. We work to do this by providing high-quality,?comprehensive adult and family medicine, pediatric health care, dental care, optometry, podiatry, behavioral health services, and pharmacies at two locations while also being a space where medical care is our central but not the sole aim. We aim to be a place where community members also receive assistance on things that will make our community socially, economically, emotionally, and physically healthy.

POSITION SUMMARY

Baywell Health is dedicated to eliminating the prevalence of infectious diseases by routine screening, education, and linking patients to care through evidence-based practices. The Community Health Advocate III manages Baywell Health's Community Need Response Program. The CNR program manager will be responsible for developing outreach and implementation practices to identify populations within our community with infectious and chronic disease management needs and to immediately connect them to tailored care plans delivered by provider teams in our clinical settings. Examples of needs we expect to address are HIV prevention and care management; Hepatitis C management, asthma, and STI's amongst others. This position will work closely with other Community Health Advocates, Patient Care Service staff, and Medical Providers to find and link newly and previously diagnosed individuals with infectious and chronic disease management need into primary medical and prevention services. On a regular basis, meets with clients to provide support, referrals, and access to services and care. Meets clients in the field and accompany clients to appointments as needed. Participates in team meetings, case conferences, and trainings as needed. Collects statistical data and maintains documentation of client files as required.

EXAMPLE OF DUTIES:

* Develop and implement community health programs. Will be involved in the policy development, planning, and implementation of health programs that respond to emerging community needs by providing health education, promoting healthy behaviors, and informing healthcare seeking practices.

* Engages with community members while implementing health programs by encouraging active participation from individuals and requesting feedback for continuous improvement.

* Coordinate with the medical staff and care teams to ensure that case management services are provided to patients with complex medical and/or psychosocial problems while ensuring health professionals provide culturally sensitive care (inpatient/outpatient referrals, appointments, scrubbing of charts, care plans, etc).

* Responsible for community outreach to find and link individuals with infectious and chronic disease management needs into care, providing follow-up, ensuring adherence to therapy, and educating and supporting the individuals.

* Conducts outreach to target populations to bring newly diagnosed and out-of- care clients to services.

* Follows up on all referrals received from internal staff and outside agencies.

* Develops plan for each patient to enter infectious disease-oriented Primary Care both internally and externally

* Conducts weekly/regular calls to check-in with client for encouragement, to provide appointment reminders, and to reschedule missed appointments.

* Coordinates transportation needs. Accompanies clients to appointments at other agencies as needed.

* Conducts treatment adherence education and support in collaboration with the patient's

medical provider.

* Provides infectious and chronic disease management navigation assessments and support. Assists with pre-authorization of clinical treatment and prescriptions for patients.

* Participates in staff team meetings, case conferences and training.

* Develops and maintains partnerships with other local, community-based providers serving individuals diagnosed with infectious or chronic disease-related needs. Will attend meetings with county-wide agencies to coordinate health services for low-income families.

* Develops and leads necessary programmatic tasks, including presentation of reports, completing monthly data reports and quarterly reports required by the funding agency as needed.

* Manages program infrastructure to develop and track CNR program outreach and outcome metrics.

* A working knowledge of current best practices, concepts, and effective models to help community members navigate infectious and chronic disease management navigation is required.

* Provides outreach/testing/counseling/education

* Provides sensitive care delivery education to Baywell providers on best practices to treat and deliver care to vulnerable populations with infectious or chronic disease needs.

* Perform other duties as assigned by supervisor.

Qualifications

QUALIFICATIONS

* Bachelor's degree in Healthcare, Social Welfare, Psychology, Public Health or a related field or an equivalent combination of education and experience.

* Minimum of two years' experience providing infectious and/or chronic disease management-related services, including experience working within underserved communities in a culturally responsive manner.

* Advocacy and outreach skills will be essential in order to promote health equity by implementing health programs and health policies that will benefit the community's health through motivational training.

* All CHA's will participate in training to become certified Community Health Workers (CHWs). All CHA's will participate in training to become Certified Enrollment Counselors. CHA's will be expected to seek and complete other health professional development training courses as appropriate.

* Good foundation of public health knowledge as it relates to healthy equity, health education, social determinants of health, chronic disease management and prevention (hypertension, diabetes, HIV, Hep C, etc.).

* Prior work related experience with HCV+ populations, active substance users, homeless individuals, transgender people, MSM and other marginalized groups.Access to reliable transportation. Willingness and ability to travel to different council clinics to perform work.

* Speak, read, write and comprehend fluent English.

* Proficient in Microsoft Office Suite with preferred knowledge of Electronic Health Records, and professional phone etiquette.

* Demonstrates effective computer, clerical, and interpersonal skills within the first 90 days of hire.

* Ability to remain calm and courteous in a high stress environment and in dealing with the general public.

* Ability to work independently with a minimum of direction.

* Excellent oral and written communication skills.

* Excellent interpersonal skills and time management skills, ability to work effectively in a fast-paced environment, with rapidly shifting priorities and competing demand.

* Excellent organizational skills, excellent attention to detail and ability to provide timely follow-up.

GENERAL REQUIREMENTS
