JOB DESCRIPTION

This salary range may be inclusive of several career levels at Valley Medical Center and will be narrowed during the interview process based on several factors, including (but not limited to) the candidate's experience, qualifications, location, and internal equity.

TITLE: Community Health and Wellness Manager

JOB OVERVIEW: Oversees the planning, development, implementation, and management of community-wide well-being outreach initiatives. Develops virtual and in-person programs and communications that create awareness for Valley services and clinics, builds motivation to live a healthy lifestyle, and provide tools/education that can help community members proactively maintain their health. Responsible for community related wellness education and communication. The position requires critical thinking skills and the ability to competently work independently with a variety of departments, clinics, and community groups.

DEPARMTMENT: Community Outreach

WORK HOURS: Typically, Monday - Friday; Occasional weekend and/or evenings may be required.

REPORTS TO: Director, Marketing & Outreach

PREREQUISITES:

* Bachelor's degree in Nursing, Public or Community Health, Healthcare Administration, or another closely related field, required. Four years of additional relevant and applicable experience may substitute degree requirement.

* Current Washington State Nursing License required.

* Minimum three years recent clinical experience.

* Minimum four years of experience working with clients on health education, motivational change, or behavior modification in a healthcare or community service setting, required.

* Experience coordinating community outreach efforts, volunteers, and events required.

* Fluent in one or more of the most commonly spoken languages in our hospital district other than English: Spanish, Russian, Vietnamese, Punjabi, and Amharic, preferred.

QUALIFICATIONS:

* Experience developing well-being programming to help reduce health risk and encourage healthy behaviors. Have the knowledge and skills to conduct health screenings, testing, and vaccination and provide nursing care and oversight during outreach activities.

* Experience leading wellness initiatives.

* Experience working with varied and diverse organizations in a community health or population health capacity.

* Excellent written and oral communication and interpersonal skills, to internal and external audiences.

* Ability to organize, prioritize, and manage multiple projects at one time under tight deadlines.

* Ability to function within all levels of organization and in partnership with community organizations.

* Ability to develop effective content from concept to execution, depending on needs of community and community organizations.

* Exceptional follow-up and follow-through.

* Commitment to keep abreast of current national wellness and well-being initiatives and activities, developing a network of resources and contacts to use as appropriate.

* Exceptional organizational skills and ability to be nimble and flexible to accommodate community and organizational needs.

* Demonstrates excellent teaching, coaching, and motivational skills.

* Possesses the ability to tailor communication skills to diverse populations, varied age groups, and a variety of health conditions.

* CPR certification or eligible to take within 60 days of hire.

* Strong computer skills and ability to quickly learn programs related to digital communication platforms, class registration, and event management.

* Social media acumen to develop and drive healthy lifestyle content and create/maintain online event listings.

* Demonstrated ability to manage multiple tasks and demands, ensuring a high degree of accuracy and attention to detail.

* Demonstrated commitment to improving health equity in our community and sharing that knowledge with Valley staff and providers.

B. Essential Responsibilities and Competencies:

* Conducts health screenings, testing, and vaccination and provides nursing care and oversight during outreach activities.

* Partners with Lifestyle Medicine, Fitness Center, and other departments and programs at Valley to offer well-being education and health risk management that guides community members to make healthy choices and informed decisions.

* Informs the community about hospital and clinic services and health information through a variety of mediums, including written, digital, and verbal communication.

* Proactively identifies and builds relationships with a wide variety of community-based organizations to help grow Valley's outreach program and fill community health needs for the diverse demographics of our public hospital district.

* Responsible for all aspects of planning and management of wellness events and partnership for community outreach events.

* Identifies gaps in health and wellness access in our community and works to develop programming, resources, and/or events to address them.

* Responsible for maintaining class registration database, Facebook outreach event listings, and organizational outreach calendar.

* Positively and professionally represents the entire organization, including hospital and clinics.

* Works collaboratively with local community agencies and marketing department to promote and host events.

* Guides Valley social media and digital strategy as a medium to share health and well-being resources. Analyzes metrics to identify successful content and improve reach to our community.

* Identifies and orders needed supplies and promotional items.

* Serves as lead in recruiting and coordinating volunteers for community outreach events.

* Assists with community sponsorships.

* Prioritizes community outreach and education based on needs identified by Community Health Needs Assessment and Valley Board.

Technical Skills

* Understands presentation equipment/needs, including PowerPoint, laptop/projector, wireless mic, webinars, live streaming, etc.

* Video recording and editing equipment/software and photography skills.

* Solid organizational skills to coordinate event planning, execution, and event management process.

* Identifies community outreach opportunities by working closely with the communities and organizations within VMC's service area.

* Demonstrates solid understanding of the healthcare climate, health equity, and their role and impact in the community.
