MAJOR FUNCTIONS

Perform planning, research, and analysis. Using demographic and other data and resources, project the future human services need of the citizens of Polk County and plans how the Division can provide or arrange for the services and resources to help meet those needs. Communicates findings to alert leadership and the community to future human services needs in Polk County.

Under the supervision of the Grants & Community Health Investment Manager, provides management oversight to special projects as assigned.

ILLUSTRATIVE DUTIES

Collects, organizes, analyzes, and presents information which supports decision making and program development to meet goals and objectives of the Division. Identifies significant gaps in human service availability for citizens who are unable to secure affordable medical care, housing, transportation, and other social service needs. Establishes benchmarks and develops forecasts in collaboration with appropriate human service partners. Prepares program plans, evaluations, and reports.

Prepares county comprehensive planning elements and amendments for human services as appropriate. Stays current on planning methods, trends, and issues related to human services as an industry.

Assists with developing and managing new community health projects including ensuring all timelines and budgets are being adhered to during implementation. Prepares end-of-project analyses including programmatic and budgetary evaluations.

Provides day to day oversight of assigned projects including,

* Develop and implement quality assurance procedures

* Develop time schedules for goals of the project

* Create project procedures to meet timeline or budget restraints

* Budget monitoring during a project

* Keeping documentation during all steps of completing a project

* Conduct project reviews and periodic update checks with various project staff members to ensure projects are completed on schedule and within budget

restraints.

KNOWLEDGE, SKILLS, & ABILITIES

Knowledge of human services planning, principles, practices, and resources. Knowledge of planning and socioeconomic data collections methods. Knowledge of human services institutions and research methods. Knowledge of the implications of socio-economic conditions as they affect human service providers and their customers.

Must be results oriented and have the ability to troubleshoot and problem solve using critical thinking. Ability to assist with research, investigate, detect, verify, recognize, and formulate recommendations, solutions or alternatives based on independent analysis. Ability to perform independently or as a member of multiple team efforts across diverse groups. Ability to manage multiple priorities and meet deadlines in a demanding, fast-paced, rapidly changing environment. Must be proficient with Microsoft Office products (including Word, Excel, PowerPoint, Outlook) and possess the ability to learn and use new software or other program applications.

Ability to communicate effectively orally and in writing with professional, administrative, and community representatives representing diverse cultures and backgrounds and function calmly in situations which require a high degree of sensitivity, tact, and diplomacy. Ability to network and build relationships within the community to achieve project success. Ability to establish and maintain an effective working relationship with employees, officials, and the public. Must be self-motivated. Demonstrates personal organization, self-discipline, dependability, and ethical behavior. Maintains confidentiality of all records as required by various laws and statutes, including the Health Insurance Portability and Accountability Act of 1996 (HIPAA) as amended.

Must have the ability to see and write through repetitious use of hands to operate computer and printer/copier. Occasional standing, bending, walking, squatting, and twisting of neck. Reaching above and beyond shoulder, lifting and carrying filing weighing up to 10 pounds.

MINIMUM QUALIFICATIONS

Graduate of an accredited college or university with a master's degree in Community Health Planning, Public Health Administration, Public Health, or a related field, and have three (3) years of experience with health care programs.

Or

Bachelor's degree with major course work in Community Health Planning, Public Health Administration, Public Health, or a related field, and have five (5) years of experience with health care programs.

Must possess a valid driver's license and be able to secure a valid Florida driver's license at the time of employment.

A comparable amount of related training and experience may be substituted for the minimum qualifications except for the required college degree.

SPECIAL REQUIREMENTS

Must be willing to work beyond normal working hours when necessary. When circumstances warrant, such as a time of impending or declared disaster (i.e. hurricanes, tornados, flooding, etc.), the person filling this position will be expected to be available to participate in the Polk County Emergency Management Plan. The Emergency Management duties may require the employee to be away from their home and family for several consecutive days.

This position may be required to report for work when a declaration of emergency has been declared in Polk County.

Must undergo a Level 2 (FDLE) screening submitted electronically before commencing work. Those obtaining this certification must maintain certification as a condition of employment.
