**Position Summary**
The Goal of the program is to remove barriers that our members face due to Social Determinants of Health issues, access to care and how to access needed services for their health care needs.
This will be done in a linguistically, culturally appropriate manner.
Community Health Care Workers (CHW) serve the community as a bridge between the member (community) and the healthcare system (providers) through outreach and education.
By providing these services, CHWs help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties to become more acculturated with each other. This position is part of a new initiative in addressing members needs who are most vulnerable.
This role requires a candidate to reside in Queens or the Bronx in order to conduct face-to-face visits with members residing in Queens or the Bronx.
**Required Qualifications**
- 2 plus years experience providing clinical services; experience in community/outpatient setting as well as Public Health
**Preferred Qualifications**
- Bi-Lingual (Mandarin, Cantonese, Korean, or Vietnamese)
- Experience in serving the Medicare Populations in urban or rural environments with familiarity of resources within
- Preferred skills: Health Care Condition knowledge like Diabetes, Hypertension, Cardiovascular Disease
- Experience using a computer including the use of Excel and Electronic Health Records
- Bachelors in Community Health and/ or Social Work
**Education**
High School Diploma or equivalent G.E.D.
**Pay Range**
The typical pay range for this role is:
$18.50 - $38.82
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
