Position Summary:The Community Health Navigator works in collaboration withallmembers of the health care team. Focuses primarily on supporting individuals recently served in the SVMC Emergency Department. Utilizes appropriate screening tools to identify needs and link patient to services. Develops care initiatives in partnership with team members. Accepts referrals for the navigation of individuals in recovery to services, resources, social supports (i.e.-transportation, food, housing, employment, etc), and or other community resources as appropriate.. Serves asanadvocate and assists in identification and improvement of service delivery. This position requires expertise in care coordination, case management, knowledge of social determinants of health.Essential Functions:To accomplish this job successfully, an individual must be able to perform, with or without reasonable accommodation, each essential function satisfactorily.
