Job Overview - Community Health Nurse (RN): Join our healthcare client in New York, NY, as a Community Health Nurse (RN), providing skilled care and clinical assessments to PACE members while improving community health outcomes and access to essential services. Compensation: $58/hour Location: New York, NY Schedule: Monday to Friday, 8:30 AM to 5:30 PM Setting: Fieldwork (Will cover all of Manhattan)Responsibilities as the Community Health Nurse (RN):
- Patient Care: Provide nursing care using the nursing process, including administering treatments and medications.
- Documentation: Conduct patient assessments, complete accurate UAS-NY documentation, and collaborate on care plans with interdisciplinary teams.
- Coordination: Coordinate care across multiple settings, including home visits, PACE centers, and hospitals.
- Counseling: Educate patients and families on health, medication, and treatment plans.
- Compliance: Ensure adherence to safety standards and complete all documentation on time.
Qualifications for the Community Health Nurse (RN):
- Education: Degree from an NLN accredited nursing school required, BSN preferred.
- Experience: 1+ year in Clinical Nursing (CHHA, acute care, medical-surgical, or critical care) and 1 year working with elderly or frail populations.
- Licensure: Registered Nurse (RN) with active New York State license.
- Skills: Proficiency in UAS-NY, strong organizational and communication skills, and proficiency in Microsoft Office.
- Other: Driver s license, insurance, and car required.
