Responsibilities:

$1,500 Sign-On Bonus offered for qualified new hires. Per policy, current employees are not eligible.

As part of a multidisciplinary community health team, the Community Health Social Worker is responsible for providing outreach and education regarding mental health and social topics such as stress, depression, anxiety and coping skills to serve the needs of at risk community members, including those with chronic disease.

Duties include research, development, coordination, implementation of wellness programs in schools and other community settings. Provides program evaluation and qualitative and quantitative data collection and reporting. For grant-funded projects related to wellness, this individual will be tasked with understanding, reviewing and implementing program to ensure expenses are within budget as well as reporting outcomes. Integrates medical, psycho-social, and resource issues that impact at risk individuals and families. Provides knowledge that is relevant to coping with undesirable conditions, reducing harm, promoting recovery and to healthful living. This individual is skilled at communicating his/her expertise with groups and individuals of any age, including teaching, facilitating groups, and ongoing small and large group presentations and trainings.

The social worker will develop partnerships among key stakeholders to work collaboratively among various health and community professionals. They must have the ability to develop rapport and engage with difficult to reach, traumatized or disenfranchised individuals and/or groups. This individual must be skilled at managing multiple priorities at once and engaging in creative and compassionate problem-solving. This individual will be responsible for supervising and directing MSW interns.

This position requires the full understanding and active participation in fulfilling the Mission of the Organization. Must be flexible to meet ongoing business changes of the organizational environment and community need. Performs other duties as assigned or required.

Qualifications:

Experience:

  • Must include at least 2 years of post-graduate experience in implementation and management of health-related programs. Preferred - Experience working with children.
  • Networking experience with community based organizations desired.

Education, Licensure, Certifications:

  • Master's Degree from a School of Social work accredited by the Council of Social Work Education.
  • Current California license in Clinical Social Work Preferred.

Special Skills & Training:

  • Knowledge of community resources, trauma informed care, and adverse childhood experience is required.
  • Strong communication skills, both oral and written.
  • Significant experience and cultural competency with high-risk groups and providing services to high risk individuals and/or groups.
  • Skilled in facilitating groups and presenting to large and small audiences.
  • Bi-lingual in Spanish or one of the threshold languages of San Joaquin County preferred.
