Employment Type:Full timeShift:Description:The Coordinator provides support to the Director of Community Health and Well-Being by coordinating program services, collecting local data, ensuring its accuracy and communicating the findings to system leaders. This position is responsible for reporting on community benefit programs, supporting health equity initiatives, and supporting the development of community health needs assessments and implementation of strategies that are in compliance with regulatory and industry standards. In addition, this position supports the Community Health & Well-Being department through work on special projects as needed to achieve department and system goals.ESSENTIAL FUNCTIONS* Knows, understands, incorporates and demonstrates the Trinity Health Mission, Vision and Values in behaviors, practices and decisions.* Under Director's supervision supports the development of triennial Community Health Needs Assessment (CHNA) including but not limited to convening stakeholders, developing survey materials, collecting and analyzing data, and writing reports.* Independently develops process and tools to collect and enter Community Benefit expenditures and outcomes into the CBISA software. Reviews and assesses the data provided and seeks additional information if the data reported is inconsistent and/or does not meet community benefit guidelines.* Serves as Ministry subject matter expert on what program and activities count as Community Benefit in accordance with Catholic Health Association (CHA) and IRS regulations. Provides guidance and training to internal and external stakeholders on CHA and IRS requirements.* Provides intermediary support to internal and external stakeholders in planning, implementing, and evaluating community benefit activities (including but not limited to survey, data analysis, and interviews) and CHWB programs in response to the Community Health Needs Assessment and Trinity Health strategies (including but not limited to Health Equity, Diabetes Prevention Program, FindHelp, and SeniorFit).* Supports strategies regarding Anchor Institutions (i.e. local hiring and local sourcing) and policy, systems and environmental change initiatives in collaboration with internal and external partners.* Independently manages timely and accurate coordination for external regulatory requirements (i.e. IRS Form 990 Schedule H, federal and state requirements); populates and reviews Ministry Schedule H submission.* Works with the finance liaison to accurately calculate community benefit expenditures.* Regularly creates reports through CBISA and apprises Ministry CHWB management of community benefit spending and outcomes for budget and tracking purposes.* Partners with local RHM marketing and communications team to develop and manage media campaigns focused on community health & well-being initiatives.* Represents hospital at professional or community organization meetings and on committees at local, state and national level in order to assist in developing new programs and policies for improved delivery of health services those who are poor and vulnerable.* Assures that appropriate or required program documentation is maintained for accurate reporting of the programs and/or state and federal grant application. Assures requirements for data collection include meaningful assessments of the programs provided to the community.* Maintains a working knowledge of applicable Federal, State and local laws and regulations, the Trinity Health Integrity and Compliance Program and Code of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior.* Performs other duties as assigned in relation to Community Health and Well-Being.MINIMUM QUALIFICATIONSBachelor's degree in health care administration, public health, community health or a related field with two (2) years of experience coordinating programs and managing projects within community health, public health, or social services; or an equivalent a combination of education and experience. Must be able to operate effectively in a collaborative, shared leadership environment. Must possess a personal presence that is characterized by a sense of honesty, integrity and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals and values of Trinity Health. Strong data entry and analytics skills. Project management skills. Excellent communication (interpersonal, verbal and written) skills.PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONSMust be able to adapt to frequently changing work priorities and be able to prioritize and balance the requirements of working with the System Office and Regional Health Ministries (RHMs).
