SUMMARY

The school-based community health worker (CHW) will work to expand and support social services for parents/ caregivers of chronically absent students at Daniel Webster School 46. This will include informal coaching, education, and advocacy alongside parents/ caregivers, as well as appropriate referrals to available service providers and community resources. This position is funded for one year through a prevention grant to MCCOY from the Indiana Criminal Justice Institute.

ESSENTIAL DUTIES AND RESPONSIBILITIES or ESSENTIAL JOB FUNCTIONS

* Attend all required trainings and group meetings

* Maintain an accurate written record of connections and meetings with families

* Coordinate with parents/ caregivers, school staff, and service providers and maintain open and clear communication

* Maintain and update an inventory of project partners and the services they provide

* Develop and implement family engagement events and activities with school staff and service providers

* Seek out and provide support of up to 10 parents/ caregivers of chronically absent students identified by school staff at any given time throughout project period

EDUCATION AND EXPERIENCE QUALIFICATIONS or COMPETENCIES

* Must be 18 years or older

* Must have a high school diploma or GED equivalency

* Must be committed for the one-year term of the grant

* Must have reliable transportation

* Moderately active involvement in Near Westside community

* Currently live or has lived in the Near Westside for at least 2 years

* Ability to manage time to perform assigned duties

* Must have excellent communication skills (reading, speaking, and writing) and organization skills

* Have a positive attitude and be self-motivated

* Must be willing to consent to a criminal history background check and qualified to be hired in position

* Have knowledge or lived experience of social services

* Spanish/English speakers are highly encouraged to apply

CERTIFICATES, LICENSES, REGISTRATIONS

* Individual hired to this position must attend a 40-hour Community Health Worker Certification training, in addition to other relevant trainings required by the school and/or project partners

PHYSICAL DEMANDS or ENVIRONMENT

While performing the duties of this job, the employee is regularly required to use hands, see, talk, and hear. The employee is frequently required to stand, walk, and sit. The employee is occasionally required to reach with hands and arms, climb or balance, stoop, kneel, crouch, or crawl. The employee may frequently lift and/or move up to 25 pounds. The noise level in the work environment is moderate.
