As a Community Health Worker you will act as a frontline public health worker who is a trusted member of the community and/or has a remarkably close understanding of the culture of the community served. This trusting relationship enables the CHW to serve as a liaison between health and social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Community health workers provide outreach, education, referrals and follow up, case management, advocacy, and home visiting services. Community Health Worker will be a member of a medical care team in accordance with NCQA Patient Centered Medical Home standards.This is a full-time position working Monday through Friday 8am - 5pm. Travel up to 60% will be required to other locations as necessary. Travel within Wells county including Ossian, Markle, Bluffton, and Warren locations.If you are outgoing, enjoy helping others, and are flexible, this is the job for you!Community Health Worker Key Responsibilities:* Conduct intensive outreach efforts to uninsured and underinsured individuals and other high-risk populations living in the community to help individuals receive health care services.* Provide culturally appropriate and accessible health education and information.* Use population health tools to monitor and identity patients based on medical conditions, age, location, etc.* Help health care and social service systems become culturally relevant and responsive to the target population.* Develop and maintain a relationship with chronically ill patients, pregnant women, nursing mothers, individuals at high risk of health problems, and the elderly.* Coach patients in effective management of their chronic health conditions and self-care to develop strategies to improve their health and well-being with goal setting.* Assist patients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services.* Help people understand their health condition(s) and develop strategies to improve their health and well-being.* Provide basic health education to families on a range of topics including lead poisoning prevention, STD and HIV risk factors and measures to prevent transmission, risk factors associated with prenatal substance use (including tobacco), domestic violence, family planning, breastfeeding, and other important health topics.* Ensure parents understand the need for children to receive immunizations and regular health care and promote their understanding of normal child development.* Help families address such issues as completing high school education, selecting appropriate childcare and handling the multiple demands of work/school and child rearing.* Assist families to develop the necessary skills and social capital to improve their health status, lifestyle choices, family functioning and self-sufficiency (i.e. insurance, flood, housing, quality care, health information).* Provide informal counseling, social support, and follow-up.* Advocate for individuals and communities within the health and social service systems.* Build individual and community capacity to address the specific needs of the community while continuously expanding knowledge and understanding of community resources.* Facilitate communication and patient empowerment in interactions with health care/social service systems.* Participation in community events and/or travel to other MHS locations as needed* Participates in daily huddles with care teamWe are seeking Community Health Worker candidates with the following qualities:* High School Diploma/Equivalency required* Certified Community Health Worker preferred* Two years experience in the medical field, preferably primary care and/or behavioral health preferred.* Valid drivers license, a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance requiredWhy you should join the Meridian Health Services Team:* Unique Mission Innovative leader in integrated care and whole person health* Generous PTO and Paid Holidays* Opportunities for advancement* Comprehensive Benefit Package* 401k with Company match* Professional development funds for Licensure and Continuing Education* Personal Wellness and Financial Wellness Programs* Merit based pay compensation plans* Work-life harmonyDrug screen, TB test, extensive background check, and fingerprinting are required of all Meridian employees.
