Title: Community Health Worker- Access to Cancer ScreeningReports To: Patient NavigatorDirect Reports: NoneClassification/Category: Admin Level IWorkgroup(s): N/AWork Status: HybridStipend(s): NoneStatus: Non-Exempt, Grant FundedSummary of PositionThe Community Health Worker (CHW)is responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW supports providers and the Patient Navigator through an integrated approach to care management and community outreach. Communicate effectively. Interact in a respectful and professional manner with internal and external customers. Ability to gather and synthesize information. Must be void of any drug convictions or related charges. Able to travel to other RCCHC site. Understanding of, and sensitivity to, the diverse populations, socioeconomic, cultural and ethnic backgrounds to the communities we serve.Education: High school graduate or equivalent required; Associate Degree in Business Administration or related field preferred.Experience:* Experience working in a multi-cultural setting* Experience working in a community-based setting for at least 1 to 2 years preferred.* Basic computer skills required, electronic medical record (EMR) experience preferredCertification(s)/Licensure: Certified Nursing Assisted preferredKnowledge:* Knowledge and understanding of Federally Qualified Health Centers (FQHC's) in regard to the Federal 330 Public Health Service Act* Knowledge of computer systems and applications.Skills:* Skill in exercising a high degree of initiative, judgment, and discretion.* Skill in analyzing situations accurately and taking effective action.* Skill in establishing and maintaining effective working relationships with the medical and administrative staff, patients and the community.* Skill in planning, organizing, and prioritizing work; delegating and achieving goals and objectives.* Skill in exercising judgment and discretion in developing, interpreting, and implementing departmental policies and procedures.* Skill in identifying and resolving problems.* Skill in developing comprehensive reports.Abilities:* Ability and willingness to provide emotional support, encouragement and motivation to patients* Ability to anticipate and react calmly in emergency situations.* Ability to communicate clearly and effectively orally and in writing.* Ability to competently use Microsoft Office, including Word, PowerPoint, Excel, and appropriate organizational software.Essential Functions* Assist in completing activities around patient mailings including generating and quality checking mailed packets, receiving and responding to incoming participant telephone calls, and conducting patient reminder calls.* Provide ongoing follow-up, basic motivational interviewing and goal setting with patients/families* Follow - up with patients via phone calls, home visits and visits to other settings where patients can be found* Help patients set personal health related goals and attend appointments* Provide referrals for services to community agencies as appropriate* Help patients connect with transportation resources and provide appointment reminders in special circumstances* Exhibit excellent working relations with patients, visitors and staff* Work closely with medical providers to help ensure that patients have comprehensive and coordinated care plans* Work collaboratively with other clinical personnel assigned to the same patient* Knowledgeable about community resources appropriate to needs of patients/families* Responsible for providing consistent communication to the Patient Navigator to evaluate patient/family status, ensuring that provided information, and reports clearly describe progress* Act as a patient advocate and liaison between the patient/family and community service agencies* Record patient care management information in the EMR* Manage assigned caseload of patients* Assist participants with using My Chart to communicate with their providers.* Maintain compliance with workflows, policies and procedures as well as other activities to support PCMH model of care and accreditation.* Performs other duties as assigned.
