POSITION TITLE: Community Health Worker (Behavioral Health)

REPORTS TO: Director of Behavioral Health

CLASSIFICATION: Non- Exempt

CareSTL Health is seeking Community Health Workers to join our Behavioral Health Department! Frontline public health worker responsible for buildingtrusted relationships and partnerships in the community served; maximizing Substance Use Disorder (SUD) and mental-health related prevention efforts while coaching and empowering patients to take an active role in healthy lifestyles. In addition, the Community Health Worker effectively educates and links patients; families, communities and providers to community-based resources by coordinating with area agencies.

ESSENTIAL FUNCTIONS:

The following information is considered the definition of essential functions but does not restrict the tasks that may lie assigned. The Community 'Health Worker may be reassigned duties and responsibilities at any time due to reasonable accommodation or other staffing reasons.

* Complete and document protocol for assessing SDOH (PRAP ARE) tool for all patients seen and address barriers as well as provide resources to address patient's needs.

* Troubleshoots any barriers the patient might have in adhering to care.

* Evaluate crisis situations and apply appropriate interventions.

* Engage in the community through outreach to recruit high-risk patients.

* Serve as a liaison/link/intermediary between health/social services and the community to improve awareness of and facilitate access to SUD and mental health services and improve the quality and cultural competence of service delivery.

OTHER FUNCTIONS:

* Assist the primary care team in developing care management processes such as the use of guidelines; disease management techniques; case management, and patient education to improve self-:management of chronic disease.

* Build individual, family, and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as, community education, informal counseling, social support; and advocacy focusing on SUD prevention and treatment, mental health, stigma:; neo-natal abstinence syndrome, trauma-informed care, suicide prevention, and opioid overdose.

* Collaborate with team members to maximize prevention efforts and create solutions to overcome barriers to achieving healthcare goals.

* Connect members with transportation resources arid give appointment reminders in special circumstances. Transporting members is strictly prohibited.

* Follow-up with members via phone calls, home visits and visits to other settings where members can be reached.

* Strengthens community partnerships to better leverage SUD and mental health-related community resources and support more effective and efficient treatment and recovery support.

* Performs general support activities and carries out special projects as required.

JOB REQUIREMENTS AND QUALIFICATIONS:

Education:

* High School Diploma or GED required

* Bachelor's Degree in Social Work, Psychology, Public Health Nursing, Health Education,or a related field from an accredited school preferred.

Experience:

* Must have 2 years experience working in community health, human services, community outreach and/or healthcare.

* Desirable experience includes: customer. service, patient/peer coaching, medical patient interaction, community relations, outreach, data entry, public speaking, non-profits.

Additional Information:

* Clinic hours are Monday - Friday 8 a.m. to 5 p.m.
