The Community Health Worker (CHW) assists clients and their families with navigating and accessing community resources, especially through activities of the Prescribe for Health Program, ensuring integration with healthcare services to improve patient care and outcomes. The CHW is particularly responsible for helping clients address social and behavioral determinants of health needs that can be barriers to them achieving better overall health and wellbeing. High School graduate or equivalent required. Strong interpersonal and communication skills. Must have a vehicle valid driver's license, and proof of insurance. Must successfully complete Community Health Worker training within six months of employment (provided).
