Summary of Position
The Community Health Worker (CHW) is responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW supports providers and the Care Managers through an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health of patients and their family. CHW provides social support and informal counseling, advocates for individuals and community health needs, and provides services such as basic first aid, blood pressure, and screenings.

Population Served
Roanoke Chowan Community Health Center will provide open access, community-oriented primary care of the highest quality to all residents, migrants and seasonal farmworkers, as well as, visitors of Hertford, Bertie, Northampton, Gates, Washington and surrounding counties, regardless of their ability to pay.
Minimum Qualifications
Ability to read, write and comprehend instructions in the English language. Communicate effectively. Interact in a respectful and professional manner with internal and external customers. Ability to gather and synthesize information. Must be void of any drug convictions or related charges. Able to travel to other RCCHC site. Understanding of, and sensitivity to, the diverse populations, socioeconomic, cultural and ethnic backgrounds to the communities we serve.
Education: High school graduate or equivalent required; Associate Degree in Business Administration or related field preferred.
Experience:
* Experience working in a multi-cultural setting
* Experience working in a community-based setting for at least 1 to 2 years preferred.
* Basic computer skills required, electronic medical record (EMR) experience preferred
Certification(s)/Licensure: Certified Nursing Assistant preferred. BLS certification required. Community Health Worker certification within 6 months of hire.
Knowledge:
* Knowledge and thorough understanding of Federally Qualified Health Centers (FQHC's) in regard to the Federal 330 Public Health Service Act
* Knowledge of thorough understanding of the 340B program
* Knowledge of computer systems and applications.

Essential Functions
* Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement
* Provide ongoing follow-up, basic motivational interviewing and goal setting with patients/families
* Follow - up with patients via phone calls, home visits and visits to other settings where patients can be found
* Help patients set personal health related goals and attend appointments
* Educates patient on the proper use of the Emergency Room, and provides information for alternatives. Coaches patients in effective management of their chronic health conditions and self-care. Assists patient in understanding care plans and instructions
* Provide referrals for services to community agencies as appropriate
* Help patients connect with transportation resources and provide appointment reminders in special circumstances
* Exhibit excellent working relations with patients, visitors and staff
* Work closely with medical providers to help ensure that patients have comprehensive and coordinated care plans
* Work collaboratively with other clinical personnel assigned to the same patient
* Knowledgeable about community resources appropriate to needs of patients/families
* Responsible for providing consistent communication to the Care Manager to evaluate patient/family status, ensuring that provided information, and reports clearly describe progress
* Act as a patient advocate and liaison between the patient/family and community service agencies
* Record patient care management information in the EMR
* Attend regular staff meetings, trainings and other meetings, as requested
* Manage assigned caseload of patients
* Assist participants with using My Chart to communicate with their providers.
* Maintain compliance with workflows, policies and procedures as well as other activities to support PCMH model of care and accreditation.
