St. Vincent's is the first hospital in Fairfield County to be integrated into the Hartford HealthCare network, and is the system's second-largest hospital.*This position is grant-funded through June 2025 and a fantastic Per Diem opportunity for someone looking to accrue hours towards their Community Health Worker (CHW) licensure!Project Overview:In this project, you'll be joining a dedicated team focused on providing compassionate care patients with diabetes living in Bridgeport who have chosen to participate. Together, we aim to make a positive impact by offering interventions that provide access to healthy foods, extended time with registered dietitians for personalized nutrition, exercise, and culinary guidance, as well as increased support from Community Health Workers. Additionally, we offer motivational interviewing and training to help patients stay engaged and empowered in their health journey.The community health workers will:Support patients and their families by connecting them to resources and services that address their social influencers of healthConduct motivational interviewing and social assessments to help patients set goals and track progressProvide referrals and ensure patients are connected to essential servicesMake home visits to offer direct support and strengthen connections with patients and their families****Please include all Community Health Worker experience, paid or unpaid, in your resume****QualificationsHigh School diploma or equivalent requiredActively pursuing or in possession of a Connecticut Community Health Worker (CHW) certification required (current CT CHW Certification preferred)At least 100 hours of previous experience as a Community Health Worker, paid or unpaid, preferredBi-lingual in Spanish and/or Haitian Creole strongly preferredMust be willing and able to conduct home visits to patientsWe take great care of careers.With locations around the state, Hartford HealthCare offers exciting opportunities for career development and growth.
