Care Coordination - Promotes positive customer relations in dealing with patients, visitors, and other employees. - Interacts with clients to promote a full range of health services; determines the benefit of each health program after assessing clients' willingness to enroll in available programs. - Provide cultural and linguistic health education information and current resource materials to individuals and families. - Utilize a holistic approach (considering physical, psych/social, spiritual, educational, safety and related criteria) appropriate to the age of the clients served in the assigned service area. - Refers families to clinical, social, and financial support using an established algorithm to assess patient and family needs. - Assist patients with accessing community-based resources by supporting them with applications, transportation needs and follow up on any referrals made to ensure patient utilizes identified community based resources. - Advocate for individual and commun...Health, Worker, Behavioral Health, Emergency, Department, Healthcare, Patient
