Now interviewing for a full-time Community Health Worker - Employment Position for Marion County

Aspire Indiana Health is a nonprofit provider of comprehensive whole health services including primary medical care, behavioral health, recovery services and programs addressing the social drivers of health such as housing and employment. Aspire has health centers in four Central Indiana counties serving Hoosiers of all ages and walks of life.

Position Summary

Community Health Worker's (CHWs) are flexible members of a health care team who work with vulnerable populations to improve outcomes. The goal of the CHW is to improve the health of the community by increasing health knowledge and self-sufficiency through outreach, community education, informal counseling, social support, and advocacy.

Duties include but are not limited to: gathering the necessary information concerning the patient's strengths, skills and abilities in order to assist them in developing a specific employment related goal(s) or outcome(s), develop and implement patient centered employment plan with the direction for the patient toward the achievement of the proposed goal(s) or outcome(s), developing resumes and interview skills, providing coaching services for develpment of job skills, as well as life skills, as needed, coordinating the linkage of community resources on behalf of the patient, networking with businesses to develop job opportunities for the patient and sustaining partnerships and relationships with employers.

Education and Experience

  • High School Diploma or equivalent required; Bachelors Degree in social work, psychology, counseling or related human service field from a college/university accredited by the US Department of Education preferred
  • One (1) year of related experience preferred

Certification

  • Achieve and maintain CHW certification within 12 months of hire

  • Must be able to work independently as well as collaboratively within a team setting
  • Must exhibit ability to work with targeted population, and possess knowledge of and ability to relate to community resources
  • Ability to work well under pressure in a fast-paced, time sensitive environment with shifting priorities and multiple deadlines
  • Must possess/maintain a valid driver's license, current automobile insurance and a driving record that meets the guidelines and requirements of the organization
  • Must have reliable transportation and utilize personal vehicle to transport self/others and to attend trainings and/or meetings
