Aspire Indiana Health is a nonprofit provider of comprehensive “whole health” services including primary medical care, behavioral health, recovery services and programs addressing the social drivers of health such as housing and employment. Aspire has health centers in four Central Indiana counties serving Hoosiers of all ages and walks of life.
Position Summary
Community Health Worker (CHWs) are flexible members of a health care team who work with vulnerable populations to improve health outcomes. The goal of the CHW is to improve the health of the community by increasing health knowledge and self-sufficiency through outreach, community education, informal counseling, social support, and advocacy.
- Gather the necessary information concerning the patient's strengths, skills and abilities in order to assist the patient in developing a specific employment related goal(s) or outcome(s)
- Develop and implement patient centered employment plan with the direction for the patient toward the achievement of the proposed goal(s) or outcome(s)
- Develop resumes and interview skills with patients.
- Knowledgeable of disability related information and strategies in employment advocacy
- Provide coaching services for development of patient job skills, as well as life skills, as needed
- Coordinate the linkage of community resources on behalf of the patient
- Network with businesses to develop job opportunities for patient, sustain partnership and relations with employers
- Maintaining contact with patients by conducting home visits and making telephone calls
- Building trusting relationships through active listening
- Assessing health and social needs as identified by patient priorities
- Promoting a medical home model where CHW, healthcare team and other resources interact to meet the patient's needs for services and education
Education/Experience
- High School Diploma or equivalent required, Bachelor's Degree in social work, psychology, counseling or related human service field from a college/university accredited by the US Department of Education preferred
- OBHP and 1 year of experience preferred
Certification
- Achieve and maintain CHW certification within 12 months of hire
