  • Counsels and aids patients and their families to understand health care recommendations and options.
  • Conducts intakes and assessments of client needs in accordance with First Steps Program guidelines, which includes the comprehensive and ongoing identification of health care, social, and education needs. Advocates and refers patients to appropriate community resources.
  • Assesses and assists with non-clinical problems interfering with effective patient care under supervision. Documents progress and follows up.
  • Works closely with care teams to develop care plans for identified patients.
  • Teaches maternal and infant care related to individual and community welfare as required.
  • Assists in special studies, research programs, and resource development.
  • Documents in patient charts as appropriate and requested.
  • Participates in community outreach activities as directed.
  • Performs other duties as assigned, including supporting the CHAS Health Mission and Core Values.
Qualifications:Education/Experience: Minimum of two years of health and/or social services experience required. Bachelor's degree in a social service-related field, such as social work, behavioral sciences, psychology, child development, or mental health required.Skills: Excellent oral and written communication skills required. Compassion for patients required. Computer skills required.Ability to work under tight time constraints, handle high stress and changing priorities required. Self-motivated with ability to work independently and as part of a team.Valid driver's license and insurance required. BLS (CPR/AED) required. Commitment to supporting a safe, respectful, equitable, and inclusive environment required.Physical Demands:Must be able to move around the facility between one-third and two-thirds of the day, as well as sit at a workstation and in patient rooms. Using hands occurs over two-thirds of the day, while reaching with arms occurs less than one-third of the day. Communicating is also required over two-thirds of the day, while climbing, stooping, or crawling is minimal.Lifting up to 10 lbs. occurs about half the time, while up to and above 25 lbs. occurs only occasionally. Driving frequently is required for onsite visits.The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.
