CareStar, Inc. is currently seeking an Assessment Specialist in the Clinical department to conduct program assessments using program and CareStar identified tools and processes. The Assessment Specialist is a home-based position that works independently to manage a caseload as needed.Flex staff are employed on an 'as needed' basis. When their services are required by CareStar, Inc., the employee may work up to a maximum of 29 hours per week. Employees in this category are not eligible for benefits unless otherwise set forth in the Employee Benefit Eligibility Policy but are eligible for mileage reimbursement pursuant to CareStar policy.THE POSITIONCareStar is looking for motivated individuals who: Have an active license as RN, LSW, or LISW (preferred).Have 12 months experience in home and community-based services within the past 5 years.Possess knowledge of Medicare, Medicaid, home health care, and Ohio Home Care waiver programs (desirable).Adhere to the CareStar Rule in performance of job responsibilities.Understand and comply with CareStar Policies and Procedures.Maintain confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense. Follow the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
