POSITION SUMMARY: The CHW will provide one-on-one non-clinical interventions and support to DHR Health patients. The CHW will serve as a bridge between the patient and the social systems through outreach, education, and resource navigation. The CHW will be primarily responsible for addressing a patient's health-related social needs and additional social barriers limiting the patient's ability to obtain and reach their highest level of overall wellness possible. POSITION EDUCATION/ QUALIFICATIONS: GED required Bachelor's in psychology, public health, or a related field (preferred) Community Health Worker (Texas) Certification required Bilingual Spanish and English required JOB KNOWLEDGE/EXPERIENCE: 1 year of relevant healthcare experience required Knowledge of local community resources and healthcare services strongly preferred POSITION RESPONSIBILITES: Provide one-on-one non-clinical interventions to address patient's Social Determinants of Health in order to support the patient in overcoming barriers to care and increase the potential for a positive health outcome. Provide general education to DHR Health patients regarding social services and community resources Collaborate closely with a DHR Health interdisciplinary team to address social needs and barriers to health and wellness. Actively participate in community outreach, assist patients in navigating a variety of social services and community resources, and provide referrals based on individual needs. Gather data on patient's Health-Related Social Needs and track referrals successfully made to community resources. Maintain accurate, HIPAA Compliant, and timely documentation and records of interactions, assessments, and referrals made to community resources and social services. Provide weekly follow ups with DHR Health patients for 30-days post discharge to monitor progress and adherence to care plans. LINES OF REPSONSIBILITES: (Chain-of-command) 1. CHW ? 2.
