Assists in the coordination of care and management of caseloads including, conducting patient home visits to evaluate plan of care and ongoing community service needs. Serves as a resource to staff and others, provides a wide range of supportive office functions for the office-based team.**Location:**Home Care-Erie County**Recruiter:**Adrian Mudd**Minimum or Preferred Qualifications:**AAS - Associate of Applied Science required. BS - Bachelor of Science preferred. 2 years of experience in human services required. 2 years of experience in a homecare setting preferred.
