Job Summary:Community Health Workers (CHWs) are frontline public health workers that serve as a liaison between the provider and health/social services. CHWs are seen as trusted members of the community by building a rapport with the community members that they are serving. Historically, CHWs have been successfully utilized to help manage chronic diseases, such as high blood pressure, diabetes, and high cholesterol. Provides social support and informal counseling, advocates for individuals, conducts basic health screenings, assists with referrals to a Primary Care Provider (PCP) and other community resources, and assists with enrollments in programs. Ensures successful linkage to care, the CHWs will follow-up with the participants to address any potential barriers to care.
Job Description:Essential Duties and Responsibilities:- Assists in identifying and engaging patients who would benefit from hypertension (HTN) initiatives offered through Wayne Health and Wayne State University.
- Effectively educates, motivates, and supports patients in pursuing behavior and lifestyle modifications that improve their health status and quality of life.
- Advocates for vulnerable populations within the health care system and the community at large.
- Assists providers and Case Managers with care coordination and care transitions for project enrollees.
- Assists with system navigation and enrollment into the Bring it Down (BiD) Program, Wise Woman, LEAP Project, and all other HTN and chronic disease projects through Wayne Health and Wayne State University.
- Provides health screenings such as vital sign intake, HIV testing, Lead testing, and Point of Care testing.
- Assists enrollees in accessing health and community resources, including but not limited to: insurance coverage, transportation, food assists, and housing.
- Responsible for a large case load among all projects. CHWs are responsible for monthly remote visits and ongoing communication with enrollees.
- Assists in the collection of health data and source documentation.
- Assists with increased new patient and repeat care visits by promoting the center(s) within the community and connecting individuals to potential referral sources such as community-based organizations, primary care clinics, and other groups and/or programs.
- Educates and assists enrollees in understanding their chronic health conditions and healthcare plans.
- Attends on-going trainings and continuously expands knowledge and understanding of community resources.
- Provides accurate and timely information to the referring provider and the Case Manager (if applicable), ensures that provided information clearly describes progress.
- Assists with Patient Services Representative duties as needed.
- Instructs patient of date, arrival time, and location of appointment and answers any questions asked by the patient or representative.
- Works in teams to manually distribute paper flyers via walking to communicate and educate public about Mobile Health Unit services.
- Performs administrative duties and all other duties that are delegated to them.
Education, Licensure/Certification, and Training:- High school graduate or equivalent
- Valid driver's license. Must have reliable transportation and insurance.
- Successful completion of a Community Health Worker formal training program and certification.
- Experience with public health work within the community preferred.
Skills and Abilities:- Maintains high level of confidentiality and follow HIPAA standards to safeguard patient information.
- Demonstrates a high level of cultural competencies in the daily performance of duties.
- Ability to build and maintain relationships with a diverse population.
- Ability to multitask various roles as needed throughout the Mobile Health Unit; quality assurance and lab orders.
- Demonstrates excellent customer service performance on all contacts, with attitude and actions that are consistent with Wayne Health and Wayne State University (WSU) policy.
- Ability to work independently with minimal supervision; Works collaboratively and effectively within a team.
- Excellent interpersonal, written and oral communication skills and an ability to present self in a courteous, tactful, and professional manner.

- Computer skills i.e. Microsoft Word, Excel, Outlook, and Internet.

- Maintains a safe and clean work space and patient walk-up area at the Mobile Health Unit site.
- Efficiently types, talks and navigates multiple healthcare platforms and reference material at the same time to assist the patient in a timely manner.
- Ability to move about consistently changing environment to approach homes and patients/potential patients.
- Must be flexible to work in both indoor and outdoor settings (ex. Emergency department and community sites).
Safety Requirements:- Compliance with Department Health and Safety policies and procedures
Working Conditions/Schedule:- 40-hour work week, hours and days to be determined by the business needs of the department. Additional hours may be required. Must be flexible to work in both indoor and outdoor settings (ex. Emergency department and community sites).
- Ability to frequently lift and move Mobile Health Unit equipment weighing up to 40 lbs.
- Ability to travel to Mobile Health Unit sites and be flexible with changes in locations/sites due to external factors (i.e. weather, cancellations, add-ons, etc.).
- Ability to adapt to working in outdoor elements, rain, snow, heat, in compliance with the departments Weather Policy.
