Job Summary: Under general supervision, the Community Health Worker III will conduct wellness consultations that include Social Determinants of Health screenings, health coaching, health education, and referrals to social services available in local communities. Evaluate the effectiveness of services provided and their impact on improving overall health. Provide ongoing support to patients and their families by offering referrals to Primary Care Physicians (PCP's) and community resources.Essential Duties and Responsibilities:Conduct Social Determinants of Health (SDOH) screenings to identify individual or family needsRefer clients to community resources and other agencies to meet identified needsConduct community outreach to promote, maintain, and improve individual and community healthAssist clients in accessing community services and adopting healthy behaviorsProvide health education for chronic disease prevention (e.g., blood pressure, cholesterol, diabetes)Offer health screenings such as vital sign intake, HIV testing, Lead testing, and Point of Care testingProvide smoking cessation opportunities with clinical pharmacistDocument client engagementsFollow up with clients to assess needs and provide additional supportDetermine client eligibility for services such as financial assistance, insurance, and other programsHelp clients access healthcare services (Medicaid, family planning, medical homes, etc.)Ensure referrals are completed and resources accessed by the patientAct as a liaison between communities, individuals, and service providersAssist in client recruitmentEngage with the community to identify referral partners and strengthen partnerships with existing agenciesParticipate in continuing educationAttend community meetings and health fairs to understand community issues and inform community members about agency offeringsProvide ongoing remote support until social needs are metMay work with the Mobile Health UnitOperate office equipment and use required software applicationsPerform other assigned duties and responsibilitiesKnowledge, Skills, and Abilities:Knowledge of Social Determinants of Health and their impact on well-beingCase management and/or care coordinationLocal resources and agency programs related to health promotionWorking in agencies providing community outreach, healthcare, managed care, public/community health, or health promotion/educationProviding health coaching and educationSkill in:Operating office equipment and using required software applications including Microsoft OfficeAbility to:Maintain confidentiality and follow HIPAA standardsWork independently, problem solve, prioritize tasks, and adapt to complex situationsHave a strong desire to help othersDemonstrate strong oral and written communication skillsBuild rapport and trust with diverse populationsEducational/Previous Experience Requirements:Minimum Degree Required: High school diploma or GED equivalent3 to 4 years' experience as a Community Health Worker or equivalent combination of experience, education, and training approved by Human ResourcesLicenses/Certifications:Licenses/Certifications Required at Date of Hire: Current CHW CertificationValid driver's license and dependable transportationMust complete clinical research training within 4 months of hire, including CITI Modules, Motivational Interviewing, Implicit Bias, and PAL2Working Conditions:Hours: Normal business hours, with some additional hours as neededTravel Required: Local and in-state travel, up to 60%Working Environment: Climate-controlled office#J-18808-Ljbffr
